- Under Statutory Authority 41 U.S.C. 1901 the VA Great Lakes Acquisition Center (GLAC) intends to negotiate a sole source contract with SIEMENS CORPORATION. The Contractor shall provide all material, supplies, equipment, and qualified personnel, management, and equipment necessary for the repair of the Siemens Artis Zee Radiology Equipment as described in this final Statement of Work (SOW) document, located in Building 200, Room number C134, at the Edward Hines Jr. VA Hospital (HINES VAH), 5000 South 5th Avenue, Hines, Illinois 60141. Please see Final Statement of Work Below. This acquisition is conducted under the authority of 41 U.S.C. 1901 Simplified acquisition procedures as implemented in FAR 13.106-3(b)(3)(i), only one responsible source and no other supplies or services will satisfy agency requirements. This will be a firm fixed price contract for a One-Time Service Contract. The NAICS Code is 811210. This Notice of Intent is not a request for competitive quotes. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service to the Contracting Officer by 10:00AM (CST - Local) 11/01/2024. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Responses received will be evaluated. However, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Contracting Officer will proceed with the sole source negotiation with the Siemens Corporation. Capability statements shall be submitted by email only as a MS Word or Adobe PDF attachment to Della Bond at Della.Bond@va.gov. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. FINAL STATEMENT OF WORK Siemens Radiology Equipment Location of Service: The Contractor shall provide all labor, material, tools, travel, management, and equipment necessary for the repair of the Siemens Artis Zee Radiology Equipment as described in this final Statement of Work (SOW) document, located in Building 200, Room number C134, at the Edward Hines Jr. VA Hospital (HINES VAH), 5000 South 5th Avenue, Hines, Illinois 60141. Background/Current Condition: the Siemens Radiology equipment: is a floor to ceiling, biplane and a multi-purpose system. Currently, this piece of equipment is hard-down due to physical damage and in need of inspections/repairs. Without service/ repairs patient care will remain impacted. Attachment: - Equipment List. Serial # Manufacturer Model Description Entry Number 404983 Siemens Artis Zee Diagnostic Radiology 134483 Scope of Work: The contractor shall provide all resources necessary to accomplish the repair of the Siemens Radiology Equipment Room physical damages listed in Attachment 1. The contractor shall ensure the proper functioning and safety of the equipment at the completion of the inspections/repairs to ensure that the device is performing effectively and safely during procedures. Performance Period: Equipment should be repaired and returned as soon as possible following award of purchase order, but not to exceed 30 days. Hours of Operations Scheduled work shall be required in the performance of this contract and shall be performed during the hospital s normal business hours of 7:00 a.m. and 5:00 p.m., local time (Illinois), excluding Federal Holidays, to include any other day specifically declared by the President of the United States to be a Federal Holiday. Please see the following list below for the Federal Holidays observed by the Edward Hines VA Hospital. Federal Holidays observed by the Edward Hines Jr. VA Hospital are: New Year's Day Labor Day Independence Day Memorial Day Thanksgiving Day Christmas Day Martin L. King, Jr. Birthday Columbus Day Veterans Day Juneteenth National Independence Day Washington s Birthday When one of the above designated Federal Holidays falls on a Sunday, the following Monday shall be observed as a Federal Holiday. When a Federal Holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday by United States Government agencies. If different times shall be needed, this shall be discussed and mutually agreed upon with POC and shall still be considered normal business hours. No work by the Contractor on Federal Holidays and no work on the weekend when the Federal Holiday is on the respective Monday or Friday shall be done. The Contractor shall obtain prior approval at least 24 hours from the POC. If such work shall be at the cost of the Edward Hines VA Hospital, a separate purchase order number shall be obtained by the Contractor prior to performing any over-time work. Work shall be scheduled in such as manner as to allow as little disruption to patient services in all campus buildings affected. On completion of billable work, the contractor shall provide timely billing of items that the Edward Hines VA Hospital is responsible per the service agreement. Documentation Upon completion of work, the Contractor shall submit the report (documentation package) to the COR or designee for an Acceptance Signature. A documentation package: acceptable for Joint Commission purposes shall be maintained by the contractor and made available to the Hines VA Hospital. Required features include, at a minimum: The date of service The model The serial number The location (building and room number) of the equipment services The name of the FSE providing services The services performed and any parts replaced or repaired. Check in/ Badging Requirements Contractors shall call the Hines VA Hospital Contacting Officers Representative (COR) when arriving and shall be required to wear a fully identifiable, temporary, VA Contractor Badge at all times while on the Edward Hines VA Hospital grounds (HINES VAH). The contractor shall then wait for the COR or VA personnel to let them into the Building 200, Room number C134. This check-in is mandatory.
