- 1. The United States Army Contracting Command � Rock Island (ACC-RI) herby awards Indefinite Delivery Indefinite Quantity Contract (IDIQ), W519TC-25-D-2008, for the procurement a Medical, Vision, and Dental Insurance for MNG Students/Dependents Attending Courses in the United States, in compliance with the specifications found at Attachment 0001-Performance Work Statement (PWS) Medical Insurance Policy 2024-2027. 2. This is a single 1- year Firm Fixed Price (FFP) IDIQ contract made in accordance with (IAW) Federal Acquisition Regulation (FAR) Part 12- Acquisition of Commercial Products and FAR 13 � Simplied Acquisition Procedures as a commercial service. The Government expects to award individual task orders issued over the course of 1 ordering periods as follows: Base Ordering Period: 07 December 2024 through 06 December 2025 The Contract Line Item Numbers (CLIN) are established and fully funded in Saudi Riyal (SAR) as follows: CLIN/Type Description 0001/FFP Single Coverage 0002/FFP Married Coverage 0003/FFP Family Coverage 0004/FFP Saudi Arabia � Value Added Tax (VAT) 3. Pricing will be IAW Cigna�s Proposal, found at Attachment 0003. The maximum and minimum quantities are as follows: The maximum contract limit is a quantity of 220 insurance policies. The minimum task order limit is a quantity of 1 insurance policy. The maximum task order limit is a quantity of 130 insurance policies. 4. The guaranteed contract minimum under this IDIQ is 90 insurance policies, to be awarded in conjunction with the award of this IDIQ via task order as follows: CLIN/Type Description Qty Unit Cost Total Amount US Dollar/(SAR) 0001/FFP Single Coverage 44 Job $14,273.87/ 53,527.00 $628,050.13/ $2,355,188.00 0002/FFP Married Coverage 5 Job $32,116.53/ 120,437.00 $160,582.67/ $602,185.00 0003/FFP Family Coverage 41 Job $49,959.20/ 187,347.00 $2,048,327.20/ $7,681,227.00 0004/FFP Saudi Arabia � 1 Lot 5. The Government is under no obligation to place subsequent orders after the award of the guaranteed minimum quantities. The issuing office for this contract is ACC-RI. Only ACC-RI is authorized to place delivery orders against this contract. Delivery Orders shall be issued unilaterally. 6. Performance will be IAW Attachment 0001-Performance Work Statement (PWS) Medical Insurance Policy 2024- 2027 and Cigna Worldwide Insurance Company�s final proposal, dated 21 October 2024, is hereby incorporated at Attachment 0002. In the event of inconsistencies between the provisions of the contract and the proposal, the inconsistency shall be resolved by the following order of precedence: (1) the contract excluding the proposal then (2) the technical, past performance, and price proposal. 7. Solicitation W519TC-24-R-2075, Amendments 0001, 0002, & 0003 and Attachments are hereby incorporated into this contract in their entirety, by reference. 9. COMPLIANCE WITH LAWS. Cigna Worldwide Insurance Company shall comply with all host nation laws, including labor laws, rules, and regulations, as well as all U.S. laws and regulations, whichever are more restrictive. 10. DIGITAL SIGNATURES. The Government may choose to use Digital Signatures generated from a Common Access Card. This has the same affect and binding authority as a hand signature. 11. This executive summary is not intended to alter the terms and conditions of the contract in any way. Any inconsistencies between this executive summary and other contract provisions shall be resolved in the favor of the other contract provisions. 12.
