- This is a Request for Information (RFI) � Sought Notice Only. This notice is issued solely for information and planning purposes only. 1. The US Government will not pay for any information or administrative costs incurred in response to this notice. Submittals will not be returned to the responder; not responding to this notice does not preclude participation in any further RFQ; IFB or RFP if issued. 2. The associated North American Industry Classification System (NAICS) is 339115 (Ophthalmic Goods Manufacturing). The business size standard is 1,000 employees. All contractors doing business with the Department of Defense shall be registered and approved through the System for Award Management (SAM). This is the Official U.S. Government system that consolidates the capabilities of CCR/FedReg, and EPLS. There is no fee to register for this site. Entities may register at no cost directly from this page. The proposed performance start date will be on or about 1 February 2025. The Solicitation number is W91YTZ-25-Q-A007. 3. Note � Prior to award your company shall reflect the NAICS Code 339115 identified on your SAM. If your company is interesting in competing on this project, and have equipment, please submit the following documentation no later than 13 December 2024 10:00 AM EST, to Kimbery.r.jackson.civ@health.mil, and Mrs. Donna Blossom at donna.l.blossom.civ@health.mil Your email response shall provide the following: Can you provide supply: This is for a contract to supply surgical lenses to the WAMC Operating Room. The contractor should have a wide inventory of surgical lenses and surgical materials so that they can meet the operating room surgical needs. Ordered surgical lenses and materials may include all major manufacturer brand name surgical lenses, fully customizable gas permeable lens, and custom specialty soft surgical lenses, and ophthalmic lenses as well as any surgical materials needed to perform surgical cases such as Cataracts, ICL, Pterygium, Strabismus, Vitrectomy and more. The contractor must be capable of receiving the materials order requests electronically either via the internet or email. The contractor will fill the order appropriately, package, and mail the surgical lenses to the designated address. The Operating Room should receive the ordered surgical lenses in less than one week, ideally in 2-3 days, from the date of the order, except in special circumstances.
