Description
This is Not a Solicitation Announcement. This is a Request for Information Only. This Request for Information (RFI) is intended for information and planning purposes only at this time; and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs. Since this is Request for Information announcement, no evaluation letters and/or results will be issued to respondents. No solicitation exits. Therefore, do no request a copy of a solicitation. The Department of Veterans Affairs VISN 4, Network Contracting Office 4 (NCO 4), is seeking information and potential qualified/verified businesses capable of meeting requirement and salient characteristics below to provide the Wilmington Veterans Affairs Medical Center with three (3) High-Definition/Medical Grade Monitors. STATEMENT OF NEED Brand Name or Equal Requirement High-Definition Surgical/Medical Grade Monitors Urology Clinic Department VAMC Wilmington Scope of Work: The vendor shall provide all resources necessary to accomplish the deliverables described in this statement of need (SON), except as may otherwise be specified. The contractor shall provide three (3) High-Definition Monitors Brand Name or Equal to: OEV-262H 26-inch LCD monitor with LED backlighting for an optimal viewing experience compatible with the Olympus brand endoscopy system. Background: The Urology Department has initiated this equipment request. The existing monitors are aging with one monitor (EE29276) that is 10 years old and will not receive any images and is not cost efficient to repair. Justification is self-evident as the monitors are needed for surgeries and to ensure proper Veteran care. General Requirement: The Wilmington VA Urology Department requires 3 brand-name or equal to Olympus OEV-262H High-Definition Surgical Monitors. Salient Characteristics & Main Features: 26-inch Full HD LCD Surgical Monitor with LED backlight 16:9 aspect ratio High brightness, high contrast, and high gradation images FLIP functions - mirror and 180° rotation Multi-modality display capability, including Picture in Picture (PIP), Picture out Picture (POP), and Clone Out1 with various image size combinations. Various input/output terminals - 3G/HD/SD SDI (×2), DVI-I (×2), HD15, Y/C, and VIDEO Eco-friendly modes such as low power consumption, power saving modes Advanced Image Enhancer) which supports the image quality by increasing image resolution and reducing image noise, enhancing the viewing experience of the Olympus endoscopy system. Display device a-Si TFT active matrix. Resolution 1920 x 1080 dots (Full HD) Contrast 1400:1 Aspect ratio 16:9 Viewing angle 178 degrees Number of colors > 1 billion Power supply for the LCD monitor for AC is 100-240V, 50/60hz. Input for input to video/HD/SD is BNC connectors. Output BNC connectors with active- through output. Remote RS-232C connector - D-sub-9-pin Compatibility multi-modality display capability, including Picture in Picture (PIP), Picture out Picture (POP), and Clone Out1 with various image size combinations. Must meet Industry Standard IEC 60601-1 and product safety standards in the U.S.A. Warranty - One-year parts and labor with ongoing support on all equipment. Patented and HIPAA compliant patient database with merge and sync capabilities across all systems. Delivery: Wilmington VAMC 1601 Kirkwood Hwy OR/Urology Department Wilmington, DE 19805 Delivery shall be made during normal Wilmington AMC warehouse business hours excluding all federal holidays. Warehouse business hours are Monday Friday, 7:00 am 3:30pm. -------------------------------------------------------------------------------------------------------------------------- Responses to this RFI should include company name, address, point of contact, phone number, and point of contact e-mail, UEI Number, Cage Code, size of business pursuant to North American Industry Classification code (NAICS) 339112 - Surgical and Medical Instrument Manufacturing, The Small Business Size Standard for this NAICS code is 1,000 employees. Please answer the following questions: (1) Please indicate the size status and representations of your business� in accordance with the NAICS code 339112, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB),� Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.) (2)� Is the required equipment available on a Federal Supply Schedule or Government Wide� Acquisition Contract?� If so, please include the schedule number.� (3) Are you the manufacturer, distributor, or an equivalent solution to the items being referenced above?� If not,� provide additional information shown below.� This is to confirm compliance with the non-manufacturer rule IAW 13 CFR 121.406(b) Nonmanufacturers.� Does� your company� exceed 500 employees� Primarily engaged in the retail or wholesale trade and normally sells the type of item being supplied;� � � Take ownership or possession of the item(s) with its personnel, equipment or facilities in a manner consistent with industry practice;� � Provide the items of a small business (4) If your company is a� large business, do you have any� authorized small business� distributors? If so, please provide their company name, telephone, point of Contact and size status (if available).� (5) If your company is� a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified?� (6) Is the item you are supplying made or manufactured in America? This is to ensure comliance with FAR 52.225-1 Buy American-Supplies. (7) If you intend to� subcontract� any work on this contract, what portion of the total cost will be self-performed/will be performed by your organization? Please provide estimated detailed percentage breakdowns related to sub contracted work and completion of job.� (8) General pricing of your products/solution is encouraged. Pricing will be used for the purpose of market research only. It will not be used to evaluate for any type of award.� (9) Please submit your capabilities� in regard to� the salient characteristics detailed above and any information pertaining to equal to items to establish capabilities for planning purposes?� (10) Please review salient characteristics/statement of work (if applicable) and provide feedback or suggestions. If none, please reply as N/A.� (11) Please provide your UEI number.� *** Submissions addressing number (10) should show clear, compelling and convincing*** evidence that all equal to items"" meet all the salient characteristics.� This� RFI� will be conducted in accordance with Federal Acquisition Regulation (FAR) Part 13.
