- AMENDMENT #1 TO REMOVE SMALL BUSINESS SET ASIDE REQUIREMENT AND ADD QA CODE 4.43 AS NOTED BELOW: �Products that are not purchased directly from the Original Equipment Manufacturer (OEM)/Original Component Manufacturer (OCM) must be purchased only from a franchised/licensed distributor of the product being offered. The distributor must provide documentation that the distributor is contractually authorized by the manufacturer as a franchised / licensed distributor along with the geographic region(s) covered by the authorization. The distributor must not use brokers (any company, person, or entity who is not an OEM/OCM or authorized by the OEM/OCM) for the purchase of items, unless pre-approval has been granted by Brookhaven Science Associates (BSA). Items to be quoted (NO SUBSTITUTIONS): Qty 1 #RGV100BL-S/High Speed Precision Rotary Stage, Brushless Motor, Direct Encoder, 360� Qty 1 #XPS-D8/High Performance Universal Motion Controller, 8-axis, DC brush, stepper, linear and piezo motor control, Ethernet, GPIO, PCO Qty 1 #XPS-DRV11/Universal digital driver card for Stepper, DC, Brushless and Direct Motors Qty 1 #XPS-DK21/Cable Kit for RGV100HL-S and RGV100BL-S motorized stages, driven by XPSDRV11. QA Paragraph(S): 3.1.4. 4.43 (See attached QA 101 for details) IMPORTANT NOTE: Freight along with Tariffs and any Packing & Handling fees are to be included in the �All-Inclusive� price of the item delivered to BNL. *The BNL PO will NOT have a line for freight or extra fees. System For Award Management (SAM): In order to support DOE reporting requirements, BSA requires all its vendors/contractors to be registered in the U. S. Government supplier database entitled System For Award Management (SAM) with their Unique Entity ID number (UEI#). Registration can be performed on line at www.sam.gov. If not already registered, please register as soon as possible since failure to do so may preclude BSA from doing business with your organization. Please note that there is no charge for SAM registration. When registering, you are strongly urged to copy and paste www.sam.gov onto your browser address rather than utilizing your search engine.
