The 99MDG Mike O Callagham Military Medical Center requires the following: 4ea AC3 Optimus Intra- Aortic Balloon Pump System FiberOptix Capability. Marathon Medical is the only SDVOSB distributor who can provide this required equipment. This notice of intent is not a request for competitive proposals and no solicitation document exists for the requirement.. Sources interested in responding to this notice are required to submit a capability statement that includes technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to meet the requirement. Capability statements shall not exceed 5 (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the closing date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order. The following is a statement of the contractors unique qualifications and why the nature of this acquisition requires the use of the authorities cited above: Arrow AC3 Optimus is the newest solution on the market; manufactured by Teleflex. Due to the proprietary nature of the real-time WAVE Algorithm, no other brand of equipmentcan provides the required IABP. The main feature of this product is the Pro Active Counter Pulsation system enabling 98% inflation timing accuracy during IABP therapy, even when patients have severe arrhythmias. This provides the unique ability of the AutoCAT2 WAVE system with FiberOptix catheter technology to proactively anticipate individual arterial pressure closures before they occur, and provide accurate inflation timing, as well as triggering even during severe arrhythmias. The FiberOptix catheter captures and transmits the high-fidelity arterial pressure signal at the speed of light, overcoming the delays seen with traditional fluid-filled arterial pressure (AP) signal systems. The proprietary real-time WAVE Algorithm is the brain of the pump-it calculates aortic flow based on the AP signal for each beat (using real-time data). This enables it to capture every single heartbeat and then signal the pump to inflate. In heart failure cases, precisely timed IABP is critical for generating beat-to-beat improvements in systolic and diastolic left ventricular performance. No failure of Teleflex balloon has been reported, over its lifetime. This is important because a mechanical failure of the balloon requires immediate removal, which can lead to serious complications caused by micro clots that form on the balloon surface, e.g.: a) peripheral thrombosis, b) myocardial ischemia, c) hemodynamic decompensation, and d) late pseudoaneurysm. Marathon Medical is the only SDVOSB distributor who can provide this required equipment. Responses are due by 4:00 PM Pacific Daylight Time, 29 August 2025. Responses shall be submitted by email ONLY as a Microsoft Word or Adobe PDF attachment to the following address(es): stephen.colton@us.af.mil.
