- SCOPE OF WORK Replace Floor OPC B8 528A5-25-513 Canandaigua VAMC 400 Fort Hill Ave, Canandaigua, NY 14424 GENERAL PROJECT WORK SCOPE DESCRIPTION Existing Conditions: The VAMC is a medical campus located at 400 fort hill avenue in Canandaigua NY including multiple multistory and historic structures featuring outpatient, inpatient, and continued living care. The OPC (outpatient care) is an active medical office with work taking place in radiology and general examination suites. Other work to be performed is on a locked floor with live-in residents. Scope of Work: The general scope of this project is to remove the existing flooring in the rooms listed (7 total) and replace it with new seamless vinyl flooring. The rooms of work to be performed may only be occupied by work one at a time and must use all measures to maintain operations. All existing equipment to remain (including but not limited to; medical equipment, casework, electrical equipment, etc.) shall be protected from damage and debris and flooring shall be installed and sealed around all items. All work areas are to be cleaned to maintain hospital cleanliness standards. Order of work areas shall be coordinated with VAMC COR as some equipment is to be removed prior to flooring removal and replaced, post install. Contractor to provide floor protection to assist equipment install. New flooring also includes integral wall base, accessories such as cove forms, caps, sealant, and termination / threshold trims. General Requirements: The contractor shall review the contract documents in their entirety prior to submitting their bid. Pay particular attention to, but not limited to, the following Division 1 specification sections. 01 00 00 General Requirements 01 32 16.15 Project Schedules 01 35 26 Safety Requirements 01 45 00 Quality Control All efforts have been made to verify conditions, Contractor is responsible for verification of all conditions and shall notify the COR of any variances from the drawings. Where incidental work is required to accomplish work shown on the drawings, this work is implied and shall be included as part of the base bid. Prior to any work commencing in an area, the contractor shall notify the COR of his intention to work in the area and shall schedule a walkthrough with the COR to determine existing conditions. After the work, the existing work areas shall be returned to their original state before final approval shall be given. After the completion of construction, the contractor shall completely clean all surfaces of construction debris. The contractor shall receive all deliveries of materials, equipment, tools, or other goods off station, and bring them to the job site. The VA shall not accept deliveries for project purposes. Any non-lead time materials will only be paid for when installed. Pre-payment of any material is at the discretion of and must be approved by the VA COR and be inspected, stored securely and insured prior to pre-payment. Any items identified as punch list items will be fixed prior to project closeout. Off tour work may be required to minimize impact to Veterans and employees. The contractor shall warranty all work including for 1 year from the date of acceptance. Warranty shall include any labor or parts necessary to ensure complete functionality of the new area. Work cleanliness must be adhered to throughout the project. Sounds and vibrations must be kept to a minimum to avoid patient disruption. The COR reserves the right to stop work if sound and vibrations are impacting patient living or employees. Submittals: The following submittals, at minimum, are to be submitted within 30 days of the notice of award. Project schedule Safety Plan Quality Control Plan Waste Management Plan Submittal Log Schedule of Values Kick-Off meeting will be scheduled 40 days after the notice of award is issued. Notice to proceed will be issued same day starting the period of performance. Rejection of any of the submittals listed above may result in delay of Kick-Off and NTP until revised submittal is received and approved by COR. The contractor shall maintain an accurate excel spreadsheet listing all submittals required. The worksheet shall be submitted by the contractor and reviewed and re-submitted as necessary. Project Duration: The total project duration for this project is not to exceed 60 days.
