- THIS IS NOT A SOLICITATION, THIS IS A SOURCES SOUGHT NOTICE ONLY and is issued in accordance with Federal Acquisition Regulation (FAR) 10 Market Research, to conduct market research. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This Sources Sought Notice is issued solely for information and planning purposes and does not constitute a solicitation or guarantee to issue a solicitation in the future. This Sources Sought Notice does not commit the Government to contract for any supply or service. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing the information provided to the Government. No contract will be awarded from this notice. The information requested in this notice will be used to determine if there are sufficient small business socioeconomic concerns that are capable of performing the work described for the requirement. Not responding to this Sources Sought Notice does not preclude participation in any future solicitation, if issued. This announcement is based on the best information available at this time and is subject to future modification. The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office (NCO) 16 is seeking sources for the following procurement: Central Arkansas Veterans Healthcare System Reference Lab Testing; Contractor shall provide reference laboratory services to the Pathology and Laboratory Medicine Service (PLMS) of the Central Arkansas Veterans Healthcare System (CAVHS), Department of Veterans Affairs Medical Center, located at 4300 West 7th Street, Little Rock, AR 72205.. Purpose and Objectives The Central Arkansas Veterans Healthcare System requires contractors to provide reference laboratory testing service in accordance with the attached Draft Statement of Work (SOW). Contractors shall provide all reference laboratory testing services listed in the Draft SOW, in accordance with all applicable federal, state and local regulations, laws, and ordinances, and in accordance with the specifications outlined below for a limited range of referral laboratory services, to include patient testing and courier services, for the Central Arkansas Veterans Healthcare System (CAVHS), Pathology and Laboratory Medicine Service (PLMS) in Little Rock, Arkansas. Services shall include the performance of analytical testing as defined by the Contractor's reference test manual, and the reporting of analytical test results and consultative services as required to assimilate the scope of its laboratory operations to CAVHS. This is the only information available at this time. The applicable NAICS Code is 621511 Medical Laboratories Size Standard - $41.5 Million Product Service Code is Q301 Medical Reference Laboratory Testing Responses. All requirements and provisions defined in the specifications of this statement of work will apply to any laboratory, i.e., branch, division, sub-Reference Laboratory, etc. performing reference testing on behalf of the Reference Laboratory. See attached SOW for the performance requirements. RESPONSE If interested, please provide the following information in a response: 1. Name of Company 2. Point of Contact; name, phone number and email address 3. SAM UEI Number 4. Contractor s small business type, if applicable, (e.g. Service-Disabled Veteran-Owned Small Business (SDVOSB), Veteran-Owned Small Business (VOSB), 8(a), HUB-Zone, Woman Owned Small Business, Small Disadvantaged Business, or Small Business). Statement indicating whether the Contractor is considered to be small under the size standard for the NAICS code identified under this RFI 5. Capability Statement. A capability statement that provides detailed information for one or more reference contracts that demonstrates the Contractor s experience providing reference lab testing services where the Contractor collects testing specimens from a healthcare facility, transports the specimens to a remote laboratory location, performs testing on the specimens, and returns test results to the healthcare facility. The information provided in a response to this notice may be used in developing an acquisition approach. Interested parties are responsible for adequately marking any proprietary or competitive sensitive information that is provided in a response. GENERAL STATEMENTS OF CAPABILITY ARE NOT ACCEPTABLE. The Contractor must provide the following information for each reference contract that the Contractor identifies as evidence of the Contractor s capability to perform the work required by this procurement: the legal name of entity with whom the Contractor held the contract. the contract numbers. a description providing details of the specific tasks the Contractor performed under that contract. the dates during which the Contractor performed the contract. the name, phone number, and email address of a person at the entity with whom the Contractor held the contract who can verify the information the Contractor provides regarding this reference contract. If a Contractor offers to demonstrate experience performing reference lab testing services through the proposed use of subcontractors, the Contractor must identify the legal name of each subcontractor that the Contractor proposes to use and provide all of the information required above for one or more reference contracts performed by each subcontractor that the Contractor intends to use. FAILURE TO SUBMIT THE IDENTITY, LOCATION, AND CLIA CERTIFICATION NUMBER OF THE LABORATORY THE CONTRACTOR WILL USE TO PERFORM EACH TEST LISTED IN ATTACHMENT A MAY BE VIEWED BY THE GOVERNMENT AS EVIDENCE THAT THE CONTRACTOR LACKS THE ABILITY TO PROVIDE THE REFERENCE LAB TESTING SERVICES REQUIRED BY THIS PROCUREMENT. THIS, IN TURN, MAY AFFECT THE GOVERNMENT S DETERMINATION ABOUT WHETHER THE REQUIREMENTS FOR A SET-ASIDE PROCUREMENT HAVE BEEN MET. ALL of the following Testing Laboratory Information: Copy of the Contractor s CLIA Certificate of Accreditation that identifies the laboratory specialties/subspecialties that the Contractor is certified to perform and their effective date for each Contractor laboratory location at which the Contractor will perform the laboratory testing described in the Draft Statement of Work.
