- A.6 - STATEMENT OF WORK Statement of Work Project # 671-22-118 Replace Cast Iron Drain Lines and Chemical Waste System 2nd & 3rd Floor South Texas Veterans Health Care System (STVHCS) Audie Murphy VA Medical Center 7400 Merton Minter Blvd. San Antonio, Texas 78229 Background: The South Texas Veterans Health Care System, Audie Murphy Hospital has a requirement to replace cast iron drain lines and the chemical waste system for the 2nd and 3rd floors of Audie Murphy VA Medical Center. Current Conditions: The current drain lines are outdated and leaking throughout the hospital. The replacement of Cast Iron Drain Lines and Chemical Waste System for the 2nd and 3rd Floor is necessary to upgrade this antiquated system. Scope of Work: The Contractor shall provide all labor, materials, supplies, tools, equipment, supervision and construction services necessary for repairing and replacing sanitary, waste and chemical drain lines and all other incidentals necessary in accordance with this Statement of Work. The Contractor shall execute the work in accordance with the design plans and specifications. The objective is to deliver a complete and functional reclaimed area and system to the government. All work shall be carried out in the 2nd and 3rd floor interstitial space, therefore some work shall be off the catwalk and shall require special harness equipment to attach to the structural beams above. Follow Occupational Safety and Health Administration (OSHA) Part 1910.2A. All construction and installation work shall be in compliance with the VA TIL, VA Plumbing manual specifications for material and workmanship. Additionally, the Uniform Plumbing Code (UPC), International Plumbing Code (IPC), and Cast-Iron Soil Pipe Institute (CISPI) and all applicable city, county, state, and federal regulations and OSHA standards. Refer to the construction drawings and specifications for requirements. 3. Site Visit: A site visit will be scheduled for contractors to familiarize themselves with all existing site conditions. Although a site visit is not mandatory, it is highly encouraged for all interested contractors to attend. Refer to the solicitation, FAR provision 52.236-27, Site Visit (Construction), for details regarding the site visit. 4. Description of Tasks The Contractor shall obtain a 'Permit for Interstitial Entry' prior to the commencement of any work tasks. The Contractor shall protect the ceiling and occupants below from construction debris prior to commencement of work. The Contractor shall identify all sanitary and waste fixtures, floor drains, and floor sinks, including those with drains under the slab, that are to be repaired or replaced as per the drawings. The Contractor shall identify and secure all fixtures and drains prior to removal and replacement of branches, laterals and p-traps. The Contractor shall take protective measures to prevent overspills and damage to piping, materials. The Contractor shall perform all work activities without disrupting the duties of staff or the care of patients. The Contractor shall ensure fixtures are locked out tagged out prior to the removal and replacement of piping. The Contractor shall follow the work task for the removal and replacement of cast iron piping and associated rods and hangers, as outlined in the GENERAL NOTES on the drawings and as shown on the plans. The Contractor shall demolish and remove all existing piping, fittings, and associated hangers as shown and specified in the GENERAL NOTES on the drawings. All existing trap primer piping shall be maintained and reused. Any damaged or missing trap primer piping shall be replaced as indicated on the GENERAL NOTES on the drawings. The Contractor shall install a new Schedule 40 service weight and an extra-heavy bell-and-spigot cast-iron pipe to match existing conditions, as shown on drawings. The Contractor shall install piping so that the manufacturer s markings and labels are clearly visible and consistently oriented along all sections of the piping run. The Contractor shall identify existing hole penetrations through mechanical walls caused by removal of existing piping during demolition. The Contractor shall then patch and seal penetrations to maintain structural integrity of the wall assembly as shown on the GENERAL NOTES on the drawings. The Contractor shall ensure that all piping is installed with the proper slope. The Contractor shall reinstall the trap-primers on all new traps. The Contractor shall perform all work as shown on the drawings and specifications for the South Texas Veterans Health Care System, Audie Murphy Hospital, San Antonio, Texas, in accordance with the Statement of Work. Replace Chemical Waste System reference P101, P102, P103 Chemical Waste System 2nd and 3rd Floor. The Contractor shall reinstall chemical piping using identical socket-type, flame-retardant, polypropylene, chemical-resistant fused pipe. The Contactor shall coordinate with the lab and office area custodians to schedule a temporary discontinuance of use of fixtures. The Contractor shall ensure fixtures are locked out tagged out prior to the removal and replacement of piping. The Contractor shall follow the work task for the removal and replacement of chemical waste piping and associated rods and hangers, as outlined in the GENERAL NOTES on the drawings and as shown on the plans. The Contractor shall demolish and remove all existing piping, fittings, and associated hangers as shown and specified in the GENERAL NOTES on the drawings. The Contractor shall identify existing hole penetrations through mechanical walls caused by removal of existing piping during demolition. The Contractor shall then patch and seal penetrations to maintain structural integrity of the wall assembly as shown on the GENERAL NOTES on the drawings. NOTE: Chemical-Waste Systems repair/replacement shall follow OSHA PPE 1910.132, OSHA 1910.2A and VA Safety guidelines. Along with demo, the Contractor shall be responsible for filling, grading, patching, and repairing areas demoed with the same material originally disturbed to return area to a complete reclaimed/established area, acceptable by the VA. 5. Submittals: The Contractor shall provide required submittals as indicated on Schedule of Material Submittals (SMS) after award and the issuance of notice to proceed. The Contractor shall submit to the Contracting Officer Representative (COR) Construction Waste Management plan, an approved location where refuse is to be dumped and manifest for hazardous materials for review and approval by VA Safety Office prior to removal from campus. The Contractor shall also submit specifications and product info for plumbing equipment. As-built drawings (redlines) shall also be listed on the SMS, which refers to locations of new plumbing fixtures which go back to the source. The contractor shall provide the number of copies for each submittal as listed in the SMS of all required submittals. The contractor shall provide material, schedule, and test results, for Government review. The contract requires that the Contractor submit a realistic project schedule showing the order in which the work will be performed under the contract. The progress schedule will be submitted to the COR for approval and approved copy shall be provided to the Contracting Officer. After approval, a copy will be returned to the Contractor. Refer to the contract s General Requirements, Section 01 32 16.15 Project Schedules, Subsection 1.5 The Complete Project Schedule Submittal, to determine the Requirements for the project schedule submittal. The Contractor shall submit a schedule of costs to the COR for approval. The approved copy shall be provided to the Contracting Officer. This schedule shall show costs by branches of work or unit of contract, as instructed by the COR. The approved schedule will be one of the factors for determining progress payments to the Contractor for work completed. 6. Deliverables: The contractor shall provide the following deliverables: As-Builts: The Contractor shall provide as-built drawings detailing the routing of new piping to new and existing connections, including tracing the paths back to their source. At the completion of work the Contractor shall notify and schedule with the Government pre-final and final inspections. Prior to the pre-final the contractor shall provide a set of redlines for Government review to be used as a mark-up set for the pre-final. C. Materials and Methods shall be in compliance with the VA s Technical Information Library (TIL) located at https://www.cfm.va.gov/til/catalog.asp, to include but not limited to TIL DIVISION 22-13-00 Facility Sanitary & Vent Piping, TIL. DIVISION 22-66-00 Chemical-Waste Systems for Laboratory and HealthCare Facilities, VA Plumbing Design Guide, UPC, IPC, CISPI and state and all applicable city, county, state, and federal regulations and codes OSHA standards. The Contractor shall comply with all construction documents and applicable Department of Veterans Affairs Master Specifications as part of this project. Refer to construction drawings and specs for the guidelines of the construction documents. 7. Performance Period: The period of performance is 300 calendar days after issuance of the Notice to Proceed. This includes material purchase. 8. Hours of Work: The Contractor shall perform all work on site between the hours of 07:00 a.m. and 4:30 p.m. (CST), Monday through Friday, except Federal Government Holidays. Contractor employees shall check in daily with the COR upon arrival and before departure. Any work that would disrupt operations at the hospital needs to be done after hours. The Contractor may submit a written request for night, weekend, or holiday work, or for work outside the project site's normal working hours. This request shall be submitted to the VA-Contracting Officer's Representative (COR) for review at least five (5) working days prior to the proposed start of such work. The COR will then forward the request to the Contracting Officer for final approval. The approval of such requests is not guaranteed. The Federal Government observes the following federal holidays: New Year's Day January 1 Birthday of Martin Luther King, Jr.� Third Monday in January Washington's Birthday Third Monday in February Third Monday in February Third Monday in February Juneteenth National Independence Day June 19 Independence Day July 4 Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11 Thanksgiving Day Fourth Thursday in November Christmas Day December 25 9. Schedule of Work: The Contractor shall notify the Contracting Officer s Representative (COR), prior to start of work. The Contractor shall schedule work through the COR. The Contractor shall coordinate work with COR NLT 3 days before starting work. Other contractors may be working in the same area performing other work. The contractor shall execute work in a manner so as not to impede other contract work. 10. Contractor Requirements Work Coordination: Arrange such phasing to ensure the accomplishment of this work in successive phases is mutually agreeable to COR. To ensure such execution, Contractor shall furnish the Contracting Officer Representative (COR) with a schedule of approximate dates on which the Contractor intends to accomplish work in each specific area of site, or portion thereof. In addition, Contractor shall notify the COR two weeks in advance of the proposed date of starting work in each specific area. Experience: Contractor shall have in depth knowledge of construction and installation of plumbing phases: rough-in, top-out and installation of fixtures. Contractor shall have previous experience working within a hospital. The Contractor shall have knowledge of and comply with all (STVHCS) Infection Control and Life Safety requirements. Security: The Contractor is responsible for ensuring that all personnel requiring unescorted access successfully complete a background investigation, including fingerprinting, prior to commencing work. Contractors shall be required to obtain and wear security badges in plain sight above the waist at all times. Contractors found without a badge shall be immediately removed from VA property by VA Police. Allow at least 30 business days per worker to obtain a badge. Security badges are obtained through coordination with the COR. Subcontractors: Subcontractor Listing: The Contractor shall submit to the Contracting Officer information of all Subcontractors to be engaged on the worksite, indicating name, address, and nature of work involved. The list of Subcontractors to perform on this contract is due no later than 15 calendar days after the pre-construction conference. This list shall be provided to the COR and the Contracting Officer. Standard Form 1414 (Statement And Acknowledgment) Within 15 calendar days after the pre-construction conference, the Contractor shall deliver to the Contracting Officer a completed Statement and Acknowledgment Form (SF 1413) for each Subcontractor, including the Subcontractor's signed and dated acknowledgment that the labor standards clauses set forth in the contract and listed on the Standard Form 1413 have been included in the subcontract. E. Key Personnel Site Superintendent. The Contractor shall designate a qualified Site Superintendent is responsible for the day-to-day on-site management and supervision of all construction activities. The Site Superintendent shall be the primary on-site point of contact for the Project Manager and shall maintain a constant on-site presence during all work activities. Project Manager. The Contractor shall designate a qualified Project Manager to serve as the single point of contact for the Client regarding all matters related to the performance of the Services. F. Training and Certifications Licensed /Bonded Plumber: The Plumber shall be a licensed and bonded Master Plumber or Journeyman Plumber. The project site plumber shall hold a current master or journeyman plumbing license and shall always supervise un-licensed (apprentice) plumbers. OSHA Certifications: The contractor shall provide a 30 -hour OSHA certification for Site Superintendent and 10- hour OSHA certifications for all other contractor personnel. Infection Control and Safety: The Construction team, subcontractor workers and COR s must attend the Infection Prevention training provided by the VA every fiscal year. All construction workers shall have a current IC training sticker attached to their badges at all times. If this requirement is not met, workers will be removed from construction site. Lockout/Tagout (LOTO): The contractor is responsible for ensuring contractor employees are trained and certified in all LOTO procedures. G. Administration and Management 1) Performance and Payment Bonds: A Performance Bond and Payment Bond are required for this project. The Contractor shall submit this requirement within 10 calendar days from the date of the Notice of Award. 2) Insurance Certificates: An Insurance Certificate is required for this project. The contractor shall submit this requirement within 10 calendar days from the date of the notice of award. 3) Reporting: Daily Progress Reports: The Contractor shall submit daily logs detailing work completed to the Contracting Officer's Representative (COR). These daily logs will be cross-referenced with payroll records. Delivery: If for any reason any deliverable cannot be delivered within the scheduled time frame, the contractor is required to notify the CO in writing including a firm commitment of when the work shall be completed. The notice shall cite the reason for delay and the impact on the installation. The contractor will provide updated lead times on these materials, as well as an updated timeline, if the delay impacts the current timeline Differing Site Conditions: Should the Contractor encounter differing site conditions, the Contractor� shall� promptly, and before the� conditions� are disturbed, give a written notice to the� Contracting Officer directly or through the COR. Asbestos: It is the responsibility of the Contractor to notify the COR and Contracting Officer promptly, and before such conditions are disrupted, of the possible presence of asbestos pursuant to the Differing Site Conditions Clause at FAR 52.236-2. Labor Disputes: The contractor shall immediately notify the Contracting Officer of any actual or potential labor dispute delaying or threatening to delay the timely performance of the contract. Accident and Incident Reporting: The Contractor shall immediately report accidents and incidents to the COR. 4) Certified Payrolls: Certified payrolls for both the Contractor and all Subcontractors must be current with the time period being invoiced. Certified payrolls shall be provided with payment applications. 5) Payments/Invoicing: Payments will be made by VA according to the contract and the value of completed work from the Contractor's approved Schedule of Costs. All invoices shall be submitted to the COR with the contracting office copied. The COR will certify payment request against the progress report and schedule of costs. The COR will provide instruction to submit the approved invoice to Tungsten. Approved invoices are electronically submitted to the Tungsten Network. All invoices shall be in compliance with FAR Clause 52.232-33 and VAAR Clause 852.232-72 for electronic submission of payment requests. 6) Changes: If a change to the contract is necessary, it will be accomplished through a written and signed modification from the Contracting Officer. The Contracting Officer is the only person with the authority to enter into, administer, or terminate contracts, and to make changes that affect the contract's terms, conditions, cost, or time. 7) Meetings and Communication: Pre-Construction Kickoff Meeting: The contractor shall attend a Pre-Construction Conference after contract is awarded and prior to issuance of the Notice to Proceed. The Government shall provide written notification to the Contractor specifying the date and time of the conference. The contractor shall attend meetings established by the COR. Meetings are to review the progress and performance of the project and or resolve technical issues or discuss any factors that might affect the project s schedule. Contractor shall also provide meeting minutes to the COR for each meeting. 8) Requirements: Submittal Register Project Schedule Schedule of Values OSHA Certifications Accident Prevention Plan Quality Control Plan Environmental Protection Plan Certification of Insurance (COI) Letters of Appointment (Project Manager) (Site Superintendent) Performance Bond/Payment Bond-Construction Subcontractor Listing / SF 1414 H. Performance and Quality Standards Project Schedule: The Contractor is required to perform in accordance with the approved project schedule. Failure of the Contractor to comply with the requirement is cause for action under FAR Clause 52.249-10 of the contract entitled Default (Fixed-Price Construction). In the event the contract is modified, such as changing the work, adding or deleting work, or changing the completion date, a revised schedule shall be submitted for COR approval and approved schedule provided to the Contracting Officer. Quality Assurance: The Contracting Officer reserves the right to conduct any inspections or perform any tests deemed necessary at any time during the execution of the contract to determine conformance with the requirements of specifications. Acceptance Criteria: The Government s acceptance will be provided by the VA upon the completion of commissioning, when all punch list items identified during the commissioning have been resolved, when the contractor provides O&M manuals and record drawings. Safety. The Contractor shall be thoroughly familiar with all rules and regulations Governing safety at the VA facility, including OSHA, EPA, etc. The Contractor shall Ensure safe work performance by employees and subcontractors. The contractor must remove employees who refuse or repeatedly fail to comply with safe work practices and standards from the site. The Contractor must remove supervisors who fail to enforce safety and health requirements from the site. OSHA: Job site safety shall comply with and to all latest OSHA standards. The work site must be maintained in compliance with all latest OSHA standards. Equipment: The Contractor shall provide all tools and equipment to properly and safely perform the services set forth within this statement of work. This shall include, but is no limited, to all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner. Lockout/Tagout (LOTO): The contractor is responsible for ensuring contractor employees are trained and certified in all LOTO procedures. Stop Work Orders: The VA Safety Officer and COR have the authority to stop work if a serious safety violation is being committed. Stop work orders issued because of the Contractor's failure to comply with safety requirements shall not be the basis for a claim for time lost or for any delays directly or indirectly attributable to such failure. 5) Controlled Access: Unauthorized access to any area other than the work area is strictly prohibited. 6) Construction Site Security: All construction materials and equipment shall be secured at the end of each workday by the Contractor as the Government shall not be responsible for any damages to equipment or materials left unsecured by the Contractor. Due to the demand for parking on site, the VA is not going to provide a laydown, but the contractor may use space within their own worksite to house construction equipment and materials. The contractor shall be allowed to store materials in this location at their own risk. They should secure their area at the end of each day as well as throughout the day to avoid unauthorized entry. This can be achieved by a contractor provided cipher lock that has key access or a standard lockset to be keyed to the VA construction access key. The COR shall direct the contractor to the VA locksmith. 7) Daily Cleanup and Maintenance: The contractor shall maintain a clean and safe work environment by performing daily cleanup of debris and waste from the job site. The VA Safety Officer and COR have the authority to stop work if a serious safety violation is being committed. 8) Disposal: The contractor is responsible for disposing of all construction materials and debris at an approved disposal site. All disposal activities, including removal and transport, shall comply with all local, state, and federal laws, regulations, and OSHA requirements. The Contractor should ensure coverage or closure of their dumpster to avoid it being filled with debris by others. The Contractor shall haul demoed items off site when their dumpster is full. 9) Protection of Surrounding Area: In performing the contract, the Contractor shall provide for protecting the lives and health of employees and other persons; preventing damage to property, materials, supplies, and equipment and avoiding work interruptions. The Contractor shall, at all times, take care to protect and preserve all existing vegetation, structures, equipment, utilities, and improvements per FAR Clause 52.236-9 Protection of Existing Vegetation, Structures, Equipment, Utilities, and Improvements and the General Conditions section of the contract. The contractor shall take all necessary precautions to prevent damage to any Government property. The Contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, cubs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the Government, upon approval of the Contracting Officer. In the event historical items are found the Contractor shall immediately contact the Contracting Officer and wait for direction on how to proceed. 11. Additional VA Requirements The VA COR shall advise the contractor of the location and processes to obtain /adhere to the following requirement as needed: A. Identification: Contractors shall be required to obtain and wear security badges in plain sight above the waist at all times. Contractors found without a badge shall be immediately removed from VA property by VA Police. Allow at least 30 business days per worker to obtain a badge. Security badges are obtained through the COR. Badging Process Step 1:� Applicants fills out the Data form, OF-306 and depending on the level of access needed may require the Self Cert. Form. Non-US citizens require extra paperwork and more time to process. Step 2:� Contractor sends the OF-306, Data and Self Cert. forms to the COR. Step 3:� The COR sponsors the applicants in USAccess system. Step 4:� � Contractor sets up fingerprint appointment in USAaccess system. Fingerprints are taken on campus at Audie Murphy, Bldg. 5. Allow 30 workings days after the contractor completed the fingerprint appointment. Contactor must notify the COR when fingerprints are complete. *** 2 forms of ID are needed at the fingerprint appointment located on the VA Audie Murphy Campus, Bldg. 5, PIV office. The Contractor shall ensure their personnel meet the identity verification standards per Clause 52.204-9 of the Federal Acquisition Regulation. B. Parking/Smoking and Prohibited Weapons: It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited on all VA Campuses. Possession of Weapons is prohibited. C. Lavatory: Contractor personnel shall only utilize the designated restroom facilities as assigned by the COR. D. Employee Conduct. The Contractor shall not engage in disrespectful communications with VA patients. Inform the COR of the situation so it can be handled by VA personnel. The Contractor shall, at all times, take any and all steps necessary to eliminate any foul language, disrespectful language, or gestures to any other personnel including other Contractors, VA personnel, visitors, VA patients, or any others on the VA campus. E. Tuberculosis (TB): Contractors team members are responsible for TB testing of all workers scheduled to be on-site at the VA prior to the start of work. On-site workers shall provide the VA Infection Control Nurse a copy of the TB test at the time of the infection control training provide by the VA. TB test shall be within 90 days prior to assignment. Coordinate with the VA COR. TB training class is free and is a 30 min video. F. Seasonal Influenza Vaccination: VHA Directive 1192.01 establishes policy and provides guidance for the prevention of seasonal influenza in VHA facilities, through the vaccination of health care personnel (HCP) VHA Directive 1192, Seasonal Influenza Prevention Program for VHA Health Care Personnel (va.gov). HCP are individuals who, during the influenza season, work in VHA locations or who come into contact with VA patients or other HCP as part of their duties. HCP include all VA contractors who are expected to perform any or all of their work at VA facilities. Contracting Officers Representative (COR) shall ensure the flu vaccination requirements outlined in this directive are expected in all HCP contacts. VA Contracting Officers Representative (COR) are responsible for on-site HCP contact and shall inform the Contractors about this directive and its requirements. G. Information Security: The contractor shall not have access to VA Desktop computers, nor shall they have access to online resources belonging to the government while conducting services. H. Privacy Officer: The contractor shall not have access to Patient Health Information (PHI) nor shall they have capability of accessing patient information during the services provided to the VA. I. Records Manager: Contractor shall comply with all applicable records management laws and regulations, as well as National Archives and Records Administration (NARA) records policies, including but not limited to the Federal Records Act (44 U.S.C. chp. 21, 29, 31, 33), NARA regulations at 36 CFR Chapter XII Subchapter B, and those policies associated with thesafeguarding of records covered by the Privacy Act of 1974 (5 U.S.C. 552a). These policies include the preservation of all records, regardless of form or characteristics, mode of transmission, or state of completion.� In accordance with 36 CFR 1222.32, all data created for Government use and delivered to, or falling under the legal control of, the Government are Federal records subject to the provisions of 44 U.S.C. chapters 21, 29, 31, and 33, the Freedom of Information Act (FOIA) (5 U.S.C. 552), as amended, and the Privacy Act of 1974 (5 U.S.C. 552a), as amended and must be managed and scheduled for disposition only as permitted by statute or regulation.� In accordance with 36 CFR 1222.32, Contractor shall maintain all records created for Government use or created in the course of performing the contract and/or delivered to, or under the legal control of the Government and must be managed in accordance with Federal law. Electronic records and associated metadata must be accompanied by sufficient technical documentation to permit understanding and use of the records and data.� STVHCS and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Records may not be removed from the legal custody of STVHCS or destroyed except for in accordance with the provisions of the agency records schedules and with the written concurrence of the Head of the Contracting Activity. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. In the event of any unlawful or accidental removal, defacing, alteration, or destruction of records, Contractor must report to STVHCS. The agency must report promptly to NARA in accordance with 36 CFR 1230.
