- Description
- (i) This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with part 12. This announcement constitutes the only solicitation. Offers are being requested and a separate written solicitation will not be issued. Solicitation number FA301626R00130002 is issued as a Request for Proposal (RFP) for Aerospace Medical Accession Standards Medical Services Support. The Air Force Recruiting Services (HQ, AFRS/RSG), Accession Medical Waiver Division (AMWD) at JBSA-Randolph requires medical services support in accordance with (IAW) the Performance Work Statement (PWS). The services will require support from Subject Matter Experts (SMEs) as Aerospace Medical Technicians, Aerospace Medicine Specialist Physician, and Clinical Psychologist; whereof, the contractor will provide the personnel, management, transportation, equipment, supplies and any items necessary to support this effort by carrying-out non-personal, performance-based services. This acquisition is set-aside for 8(a) small business concerns. This solicitation incorporates provisions and clauses by reference. The full text of provisions and clauses may be accessed electronically at www.acquisition.gov. In accordance with RFO 12.202(b)(1), sections ii-viii are detailed in the attachments. AMENDMENT 0003 - The purpose of this amendment is to - 1. Update Attachment 1. Performance Work Statement (PWS), Section 2.4.6. 2. Update Attachment 2. Schedule of Services, CLIN numbering 3001 - 3004. 3. Update Attachment 2. Schedule of Services AND Standard Form 1449 , CLIN Description 0001 - 3004. 4. Add Past Performance Questionnaire to FAR 52.212-1, section a. Submission of Offers. 5. Add Attachment 7. Questions to the Standard Form 1449, Section J. 6. Update verbiage in Attachment 3. Past Performance Questionnaire (PPQ). 7. Extend the proposal due date from 09 April 2026 10:00 am CDT to 20 Apr 2026 10:00 am CDT. 8. Update all previous FAR language to reflect the Revolutionary FAR Overhaul (RFO) language. Proposals are now due by 20 April 2026 at 1000 AM CST and shall be submitted via email to joy.grubb.1@us.af.mil and mercedes.perez.1@us.af.mil Please view attachments in their entirety.
