The Contractor shall furnish dental services at the Angell Civilian Conservation Center, Siuslaw National Forest, Yachats, Lincoln County, Oregon. This includes all supervision, labor (including any dental assistants), transportation of Contractor's personnel and incidentals, other than those provided by the Government, necessary to successfully provide dental services. Contract time is one year after receipt of the Notice to Proceed. Included is a three-year optional renewal on the contract. Estimated start work date is July 1, 1995.
