The contractor shall provide quarterly preventive maintenance visits to be performed at three month intervals. Services also include unlimited service calls which include all parts, labor, and travel. Services to be performed Monday through Friday, excluding holidays, with response time of four (4) hours by telephone and twenty-four (24) hours on-site. Annual calibration in accordance with manufacturer's requirements, Bureau of Radiological Health Standards, and NAVMED P-5132. Performance period is a base year commencing 01/01/96 through 09/30/96, with option provisions for two (2) additional one year periods commencing 10/01/96 thru 09/30/98. Planned sole source to Medasys, Inc., Ann Arbor, MI. All responsible sources may submit an offer which shall be considered by the agency. Only written or facsimile requests will be accepted.
