Provide dental hygiene services at the Coast Guard clinic located in Kodiak, AK to beneficiaries referred for treatment by Coast Guard dentists. This will be a non-personal services contract and medical liability insurance coverage is required. This contract is subject to the Service Contract Act. Contractor will work weekdays during hours established by the Clinic Supervisor, normally not to exceed 8 hours per day, 32 hours per week. Period of performance will be 01 Jul 1996 through 30 Sep 1996 with three option years. The RFP will be issued on or about 03 Apr 1996. This procurement is a 100 per cent Small Business Set-Aside. The SIC code is 8021 and the small business size standard is $5 million average annual receipts for the preceding three fiscal years.
