Evangelista, (315)477-4507 Provide prescription eyeglasses and fittings from VA prepared prescriptions throughout the VISN 2 service area. Service area consists of all of upstate New York with orders for service being placed from Albany, Buffalo, Batavia, Canandaigua, and Syracuse VA Medical Centers. Successful offeror must have the ability to provide the following: tracking and billing by ordering facility, routine prescriptions completed within 24 hours, special orders completed within five days, in-hospital service for patients unable to travel to contractor's facility, multiple locations throughout the service area, convenient hours of operation including weekend hours, on-call Optician services, on-call Optometrist services, provision of single vision, bifocal, trifocal lenses and variety of tints, dyes, photochromatics and polycarbonates. All services shall be performed on the Contractor's premises except special orders for in-hospital service. Contractor shall provide a service location within 30 miles of each of the five VA Medical Centers listed in this announcement. SIC code 3851 applies.
