The paragraph numbers below correspond to the paragraph numbers in the 041896 synopsis. Paragraphs with no changes are not included here. (i) This is amendement 0001 to solicitation DMA401-96-B-2610 for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This amendment, along with the synopsis/solicitation, dated 041896, constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. Contractors shall request a copy of the SF1449, schedule B, and the provision at FAR 52.212-3, ''Offeror Representations and Certification -Commercial Items'' via fax at (301) 227-3504. The following four items must be submitted with all bids in order to be considered: 1) A signed SF1449; 2) A completed schedule B; 3) A completed Far 52.212-3 provision; 4) A list of three references for whom the supplies required under this solicitation or similar supplies have been provided.
