(513)475-6339 Provide Architectural-Engineering (A-E) Services for
design development and construction documents for a project to expand
the current emergency room (ER) space into the current ambulance dock
area to accommodate new holding areas, cardiac emergency space,
admitting, and other support functions of the ER. The project will also
include renovation of the existing ER space to tie the existing and new
areas together at the VA Medical Center, Cincinnati, Ohio. Only firms
within a 150 mile radius of Cincinnati, Ohio will be considered. The
work may involve Design, Drafting, Planning, Studies, Site
Investigation, and Construction Phase Services associated with this
type of project. The proposed services will be procured by a negotiated
fixed-price contract. Estimated design start date is January 1997. The
estimated cost of construction is between $1,000,000 and $5,000,000.
Interested firms are invited to submit a completed Standard Form 254
''Architect-Engineer and Related Services Questionnaire,'' and Standard
Form 255 ''Architect- Engineer and Related Services Questionnaire for
Specific Project,'' to this office on or before 4:30 P.M., November 22,
1996. Those firms having the capability to provide the services shall
address the following evaluation criteria: (I) Team proposed for this
project: Background of the personnel (1. Project manager, 2. Other key
personnel, 3. Consultants); (II) Proposed management plan: Team
organization (1. Design phase, 2. Construction phase); (III) Previous
experience of team proposed for this project: Describe projects; (IV)
Location and facilities of working offices: A. Prime firm, B.
Consultants; (V) Present proposed design approach for this project: A.
Describe proposed design philosophy. B. What problems do you
anticipate and how do you propose to solve them? C. Describe possible
energy applications. D. Describe innovative approaches in production
and design.; (VI) Cost Control: A. What control techniques are planned?
B. Who will be responsible for cost control? C. Review recent projects
to demonstrate ability to meet project cost target; and any additional
construction costs caused by design deficiencies - not program
changes.; (VII) Estimating effectiveness: Review ten most recently bid
projects to demonstrate estimating effectiveness; (VIII) Schedule
Control: A. What techniques are planned to assure that schedule will be
met? B. Who will be responsible to assure that schedule will be met? C.
Review recent projects to demonstrate ability to meet project
schedules.; (IX) Describe your experience and capabilities in the
following areas: A. Specialized experience for this project, B.
Computer aided design and drafting, C. Other computer applications; (X)
Miscellaneous: A. Describe awards received for design excellence. B.
Describe circumstance and outcome of any litigation you have been
involved in over the past five years. C. Indicate type and amount of
liability insurance you carry. This is not a Request For Proposal. The
Standard Industrial Code is 8712, with an average annual sales or
receipts for the preceding three fiscal years not to exceed $2.5
