This announcement constitutes the only solicitation. A quote is being requested and a written solicitaiton will not be issued. Solicitation number F22600-97-T0405 is issued as a request for Quotation (RFQ). The standard classification code is 5122, small business size standard is 100 employees. The incorporated document and provisions and clauses are those in effect through Federal Acquisition Circular 90-42. Contract Line item 0001 is for one each Patient Prescription Interactive Information System, to be fully integrated with in place Composite Healthcare System(CHCS), MUMPS AudioFAX Inc. CHCS-S04 Interative Telephony 4 Telephone Line Computer System consisting of the following which allows for a Trade-In of the Microlog System currently in use: CHCS-U4-4 Additional Telephone Lines (8 Total); CHCS-U4-4 Additional Telephone Lines (12 Total); Rotary - Rotary Telephone Support (4 Telephone Lines); Rotary - Rotary Telephone Support (to 8 Lines Total); Rotary - Rotary Telephone Support (to 12 Lines Total);CHCS-RX/INFO-tm, Pharmacy Medication Information; CHCS-REFILL-tm, Pharmacy Inquiry and Refill; CHCS-Rx/MESSAGE, Pharmacy Message Capability; Off Line Data Base, System runs with Full functionality, even when CHCS or communicatins lines are down; Initial Consultations, 4 Lobby Phones and System for initial consultations. THe system also includes the following: fully configured DEC Pentium computer system including all related hardware and software; Micronetics Standard MUMPS multiuser license; MUMPS Talk Interactive Telephony software license and related telephonehardwae; complete system documentation; installation and training included; One year free maintenance from date of installation which includes on-site next business day hardware maintenance; Software support, updates, and enhancements; Updates to medication consultation text. The system must be capable of providing 24 hour complete medication monograph information to patients; function as a complete refill processing system during CHCS downtime; allow re-batching of prescription information into CHCS in case a prescription refill batch fails to print; provides medication counseling to patients as required by Joint Commission Accreditation of Helthcare Organization(JCAHO) and Obminbus Budget Reconcilitation ACT(OBRA) 90 mandated by Congress. Delivery shall be made within 60 days after award of purchase order. Inspection and acceptance shall be made at destination. Clause 52.212-2 Evaluation-Commercial Items(Oct 1995)--Evaluation Factors are past performance and price. The evaluation factors will be evaluated as being substantially equal. The Government reserves the right to make award on the initial offer received without discussions. The warranty period shall begin upon final acceptance of the items listed in the schedule.
