This notice will modify the original notice published on 09/11/96, for Comprehensive Medical Services: Inpatient/Outpatient Medical Services for federal prisoners at the Federal Correctional Institution in in Beaver, WV. The contractor shall furnish facilities, equipment and personnel necessary for medical care. the guaranteed minimum/estimated maximum requirements for inpatient services is as follows: 20 minimum inpatient days/ 200 maximum inpatient days for the Base Year and four possible option years. The Contractor shall provide appropritely trained and licensed physicians to perform on-site sessions at the Federal Correctional Institution/Federal Prison Camp. These onsite services shall include the following, with the guarnteed minimum/estimated maximum number of sessions for each. Orthopedic Surgeon,(session 4 hours) 6 minimum/ 48 maximum; Dermatologist, (session 4 hours) 3 minimum/ 15 maximum; Dietician, (session 3 hours) 8 minimum/ 24 maximum; Podiatrist, (session 3 hours) 8 minimum/ 24 maximum; Cardiologist (session 4 hours) 4 minimum/ 12 maximum; Urologist, (session 3 hours) 4 minimum / 10 maximum; Physical Therapist, (session 3 hours) 1 minimum/ 20 maximum; General Surgeon, (session 3 hours) 12 minimum/ 24 maximum; Ophthalmologist, (session 3 hours) 5 minimum/ 12 maximum; Neurologist, (session 4 hours) 3 minimum/ 20 maximum; Oral/Maxillofacial Surgery Services, (session 4 hours) 2 minimum/ 12 maximum; Ear, Nose and Throat (ENT) Specialist (session 4 hours) 4 minimum/ 20 maximum; Bio-Medical Services 2 visits per year minimum/ 4 vistits maximum per year; Bio-Hazard Waste Disposal, 1 pick-up per month minimum/ no more than 2 pick- ups per week. The proposed contract period will be a base year plus four (4) one-year options, for a total of five years; (Base Year: Date of contract award through 12 months; Option Year One: 13 months through 24 months; Option Year Two: 25 months through 36 months; Option Year Three: 37 months through 48 months; Option Year Four: 49 months through 60 months. The Standard Industrial Code for this acquisition is 8062. Multiple award of contracts is anticipated. Contractors are advised that award can be made without the benefit of discussions, and are encouraged to submit initial proposals to reflect all of the reqirements of the soliciation.
