modification will correct previous notice published 12/11/96. The
Federal Bureau of Prisons seeks qualified organizations capable of
providing inpatient/outpatient medical services for federal prisoners
at the United States Penitentiary/Federal Prison Camp in Terre Haute,
IN. The contractor shall furnish facilities, equipment and personnel
necessary for medical care. The guaranteed minimum/estimated maximum
requirements for inpatient services is as follows: 60 inpatient days
minimum/ 350 inpatient days maximum. The Contractor shall provide
appropriately trained and licensed physicians to perform on-site
sessions at the United States Penitentiary/Federal Prison Camp, Terre
Haute, IN. These on-site services shall include the following with the
guaranteed minimum/estimated maximum number of sessions for each.
Generally a session is equal to on (1) hour. Orthopedic Surgeon, 52
minimum sessions / 208 maximum sessions; Optometrist, 52 minimum
sessions / 208 maximum sessions; Dermatologist, 12 minimum sessions /
48 maximum sessions; Cardiologist, 30 minimum sessions / 36 maximum
sessions; Psychiatrist, 26 minimum sessions / 104 maximum sessions;
Urologist, 12 minimum sessions / 48 maximum sessions; Physical
Therapist 12 minimum sessions / 48 maximum sessions; Internist, 26
minimum sessions / 104 maximum sessions; General Surgeon, 26 minimum
sessions / 104 maximum sessions, Opthalmologist, 6 minimum sessions /
24 maximum sessions, Neurologist, 20 minimum sessions / 24 maximum
sessions, Oral Surgeon, 26 minimum sessions / 104 maximum sessions,
Ear, Nose, and Throat Specialist, 12 minimum sessions,104 maximum
sessions. Guaranteed minimum / estimated maximum number of sessions are
for the base and all option years. The proposed contract period will be
for a base year, plus four(4) one-year options, for a total of five
years; Base Year: Date of contract award through 12 months; Option Year
One: 13 months through 24 months; Option Year Two: 25 months through 36
months; Option Year Three: 37 months through 48 months; Option Year
Four: 49 months through 60 months. The Standard Industrial Code for
this acquisition is 8062. Multiple award of contracts is anticipated.
Contractors are advised that award can be made without the benefit of
discussions, and are encouraged to submit initial proposals to reflect
all of the requirements of the solicitation. This acquisition is not
