Q -- MEDICAL SERVICES SOL RFQ 618-59-97 DUE 080797 POC Janice Severs 612-725-2186 The Minneapolis VAMC intends to award contract(s) for Community Based Outpatient Clinics (CBOC) to provide Primary Care Services on an outpatient basis for approximately 200 veterans residing in the Mankato, Minnesota area and for approximately 200 veterans residing in the Owatonna, Minnesota area. The CBOC will be responsible for managing the veterans' primary medical care, including urgent and emergent care. Referrals for sub-specialty care and hospitalization will be made to the VAMC located in Minneapolis, MN. It is expected that the contract(s) will be awarded on or about October 1, 1997 with services to begin on or about December 1, 1997 for a one year period with one renewal option. Offers are being solicited on a capitated basis, fee for service or a combination of both. Responsible sources capable of meeting the Government's requirement may respond by furnishing in writing, information which substantiates the offerors ability to provide the required services. All responses must be received within 30 days of the publication of this notice. This information will be used to determine if competitive opportunities exist. Since no solicitation document exists, requests for such documents without accompanying information will be considered non-responsive to this request without further consideration.
