Phillips (205) 933-4492 The DVAMC Birmingham, AL requries full service maintenance and repair services for a Prucka Engineering 32 Channel Cardio Lab Computerized Electrophysiology System and Cardio Lab Satelite System, for the period of 10-1-97 through 9-30-98, with four (4) one year option renewal options. The Contractor will be required to perform preventative maintenance (PMI's) semi-annually. These PMI's shall be within the first five (5) working days of the months of October and April. All maintenanced is to be in accordance with manufacturer's instructions and meet all NFPA and JCAHO requirements. Service calls will be five (5) days per week, between the hours of 8:00AM and 5:00PM, Monday through Friday, Federal Holidays included. Service response time of four (4) hours from notification and be on site within eight (8) hours. Specific reporting requirements will be required and will be strictly enforced.
