Jeanelle Wright (440) 838-6091 This is a combined synopsis/solicitation
for commercial items prepared in accordance with the format in FAR
Subpart 12.6 as supplemented with additional information included in
this notice. Contractor shall provide semi-annual (2 per year)
preventative maintenance and repair of the Electron Microscope, Model
JEM-100CX, Serial #1560 75 located at the Department of Veterans
Affairs Medical Center, 10701East Boulevard, Cleveland, Ohio 44106,
Room E-08, Laboratory Service. Period of Performance shall be from
November 1, 1997 through September 30, 1998 with three (3) option
renewals as follows: Option year 1: October 1, 1998 through September
30, 1999; Option year 2: October 1, 1999 through September 30, 2000 and
Option year 3: October 1, 2000 through September 30, 2001. Contractor
shall submit pricing for all options or none. Contractor shall make all
adjustments and repairs resulting from manufacturing defects and
ordinary wear and tear (excluding externally caused damage or
negligence by the VAMC). He shall obtain all necessary licenses and or
permits required to perform this work. Contractor is responsible for
any injury to himself, his employees or others as well as for any
damage to personal or public property that occurs during the
performance of this contract by his employee's fault or negligence.
Contractor shall maintain personal liability and property damage
insurance prescribed by the Laws of the State of Ohio. All work shall
be performed by competent technicians, experienced and qualified to
work on this equipment. Certification of technicians must be submitted
with proposal. Repair of damage or malfunctioning caused by massive
power failure, accidents, negligence, misuse or abuse, weather
condition or any other not associated with the reasonable and normal
use of the equipment or originating outside of the equipment is not
required of Contractor. When Contractor is contacted by VAMC for a
emergency service call for repair of faulty equipment, the Contractor
must arrive at the medical center within 24 hours to service the
equipment and will perform all work in accordance to the manufacturer's
specifications. Contractor shall perform two (2) preventative
maintenance inspections per year to include calibration, cleaning,
lubrication, adjustments, replacement of components requiring periodic
replacement in accordance with manufacturer's specifications.
Contractor shall arrive for service calls during the administrative
working hours (Monday through Friday 8:00 a.m. -- 4:30 p.m. excluding
weekends and government holidays) Contractor shall document date of
service, date complete, model #, serial # for all service visits and
submit a legible service report to include description of malfunction,
corrective action taken and parts required to perform the service. The
parts provided must be new and have factory seals or are rebuilt to OEM
specifications. Contractor must obtain most parts within forty-eight
(48) hours. Contractor shall provide and install circuit revisions and
rectify identified Food & Drug Administration (FDA) device hazards;
provide diagnosis test equipment; test fixtures and software diagnosis
programs to input and service equipment. If a part, system component,
assembly or subsystem covered under this contract is replaced and the
replacement part is incompatible with existing equipment software, the
contractor shall provide and if necessary license the VAMC for
compatible software at no cost to the government. Contractor shall
observe equipment for relative humidity; vibration; input voltages;
fusing; circuit breakers and all other services applicable. Contractor
shall submit his proposal for base year and options in accordance to
this announcement. This announcement constitutes the only solicitation
and a written solicitation will not be issued. This solicitation is a
Request for Quotations under commercial items. This document
incorporates all applicable provisions and clauses in effect through
the Federal Acquisition Circular 90-46. Quotes must reach the
ContractingOfficer no later than October 17, 1997 at 4:30 p.m. local
time. FAR Clauses 52.212-1; 52.212-2; 52.212-4 and 52.212-5 all apply.
