62 -- REPLACE ENTRANCE AND DOCK LIGHTING SOL 541-67-98 DUE 101797 POC Jeanelle Wright, Contracting Office, (440) 838-6091 This is a combined synopsis/solicitation for commercial items prepared in accordance to the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. This Solicitation #541-67-98 is being issued as a Request for Quotations (RFQ) under commercial services and incorporates all applicable provisions and clauses in effect through the Federal Acquisition Circular 90-46. This procurement is set aside 100% for small business. The applicable SIC is 3646 and the size standard is 500 employees or less. Quotations must reach the Contracting Officer no later than 4:30 p.m. on October 17, 1997. The provision 52.212-1, Instructions to Offerors -- Commercial Items applies; 52.212-2 applies and quotations will be evaluated on price and past performance. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to this solicitation.The VAMC will award to the company offering the best overall value to the Government. Offerors shall submit with their pricing, at least three (3) business references of similar work performed and complete. Contractor is required to replace all entrance and dock lighting and refurbish and relamp one hundred and one (101) light poles located at the Department of Veterans Affairs Medical Center, 10000 Brecksville Road, Brecksville, Ohio 44141. Contractor shall supply all labor, materials, and equipment to replace all existing entrance and dock lighting with metal halide lighting fixtures of comparable light output. Contractor shall supply approx. sixty- nine (69) hubble 175 watt metal halide fixtures with bulbs, 120 volt. Part # PRS-0175H-118LL or equivalent. Seventy-seven (77) hubble 100 watt metal halide fixtures with bulbs, 120 volt. Part #NRG413H8 or equivalent. Twenty-five (25) 175 watt metal halide bulbs and twenty-five (25) 100 watt metal halide bulbs. Contractor shall schedule installation schedule two-weeks prior to beginning work. Contractor shall perform all work necessary Monday through Friday, 8:00 a.m. to 4:30 p.m. Billing for materials and installation shall be separate, Contractor may bill for materials immediately following delivery of materials to the VAMC, and installation may billed upon satisfactory completion of the work. Contractor shall also replace existing defective ingnitor boards, ballasts and relamping of all 101 fixtures. Lighting shall be inspected and repaired as necessary during the months of November, January, April and July. Contractor must provide materials and equipment to repair Light Pole #49.
