Fortuna, 860-667-6754 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation No. 689-26-98 is issued as a request for quotation. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 90-46. Standard Industrial Classification Code 8322 applies to this procurement. The small business size standard is $5 Million Dollars. The contractor shall provide all required social and financial screening services Monday through Friday from 8am -- 4:30pm at the Community Care Center, 114 Orange Avenue, West Haven, CT 06516. Performance period shall be a total of four (4) months from December 5, 1997 through April 4, 1998 at a per hour rate of $__ __ __ ___. The contractor will be required to provide psychosocial and eligibility screening to patients in community nursing homes. The contractor will also be required to provide supervision and case management and other related tasks to patients undergoing work rehabilitation within a sheltered workshop setting. Contractor shall have a minimum of a Bachelor's Degree in Social Work or a related field.
