Clemons, Contract Specialist, (90C) Bldg 218, Room 310, West Los Angeles VA Medical Center, 11301 Wilshire Blvd., Los Angeles, CA 90073 intends to negotiate a sole source procurement for Environment Of Care Mock Survey Services from Quality Healthcare Resources, Inc., Oakbrook Terrace, IL 60818. Contractor will provide one surveyor, all labor, equipment, materials and other supplies necessary to conduct a three-day Environment Of Care Mock Survey on December 9, 10, and 11, 1997. As part of the survey process, the surveyor will: (a) review appropriate station documents, (b) conduct physical building inspections with designated environment of care staff, (c) meet with designated key management staff, (d) observe a safety committee meeting, and (e) review medical center's statement of condition. In addition, the surveyor will provide a proposed agenda for December 9, 10, and 11, 1997 activities and solicit input and confirmation of agenda from the designated Contracting Officer Technical Representative (COTR). The surveyor will provide a preliminary report of all findings at an exit interview to key management staff on the final day of the mock survey. The surveyor will have met contractual obligation to the West Los Angeles VA Medical Center by completing the following: (a) conducting the mock survey and delineated above, and (b) submitting a final written report no later than December 24, 1997 to the Acting Director of the West Los Angeles VA Medical Center detailing those areas that have been found to be in compliance with JCAHO standards and also the areas with deficiencies and in need of improvement. This same written report will include suggestions and recommendations for bringing deficient areas into JCAHO compliance. The solicitation number is 691-4-98 and is issued as a Request for Quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #90-46. This solicitation is a small business set-aside and the applicable Standard Industrial Code (SIC) is 8742. FOB is destination.
