The VA Western New York Healthcare System (at Buffalo) intends to negotiate with S & W X-ray Co., located at 250 Cooper Ave., Tonawanda, NY 14150, for a full service maintenance and repair agreement of the cardiac cath lab in Room 409A. System includes Philips table, Philips overhead crane, gamtronics digital acquisition imager, Arritfchno Camera Cine. Two (2) p.m.s are required per year. The first due in 4/1/98 and the second due on 10/1/98. Service hours are Monday -- Friday 8:00a.m. to 5:00 p.m. Any unscheduled work shall be at the prevailing rate. Service engineers maintaining this system must be certified and either (1) have certification on file with the Biomed Section of the VAWNYHS, OR, (2) be able to provide certification. EXCLUSIONS: All glassware, x-ray tubes, image intensifiers, CRTs, and camera tubes are exempt, as well as damaged parts due to neglect.
