S -- UTILITIES AND HOUSEKEEPING SERVICES -- BIO-MEDICAL WASTE SOL IFB672-145-98 DUE 012198 POC Rey Clemente (787) 766-5389 The VA Medical Center, San Juan, Puerto Rico has a requirement for a service contract for the removal/disposal of Bio-Medical and Pathological hazardous waste generated at the VA Medical Center, (San Juan, PR), Mayaguez Outpatient Clinic (Mayaguez, PR), and Ponce Outpatient Clinic (Ponce, PR) on a weekly basis. This procurement is 100% set-aside for Small Business under standard industrial classification code (SIC) 4212 ($12.5M). The Government's intent is to issue a firm fixed price service contract for a base year to begin on February 1, 1998 and continue until September 30, 1998 (Base Year) with the Government having the option to extend the contract annually for four (4) additional option years subject to availability of funds. Contractor shall furnish all labor, materials, and equipment to provide disposal/recycling of non-hazardous waste in accordance with contract specifications. Bidders be considered only from regularly established business with all applicable licenses, permits, etc. from the Federal Government, Commonwealth of Puerto Rico Government, Environmental Quality Board ("Junta de Calidad Ambiental") for removal/disposal of Bio-Medical and Pathological hazardous waste. Bids received without all required applicable licenses, permits, etc. from the Federal Government, Commonwealth of Puerto Rico Government, Environmental Quality Board ("Junta de Calidad Ambiental") will be considered non-responsive.
