Main St., Fort Worth, Texas, for an exisiting primary care clinic in the Diamond Hill area of Tarrant County. Contractor shall provide capitated care to eligible VA beneficiaries. The range of primary care services includes basic primary physician care, routine labs, and basic x-ray services. The proposed contract action is for supplies or services for which the Government intends to solicit and negotiate with only one source and a solicitation is not available.
