This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information contained in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is Request for Quotation (RFQ) F08620-98-T1686. The RFQ incorporates provisions and clauses as those in effect through Federal Acquisition Circular Number 97-02. This requirement is 100% set aside for small business; the standard industrial classification code is 3679 and the small business size standard is 500 employees. Description of Requirement: Recrystal current bravo pagers for new paging system to provide Hurlburt Field AFB with digital wide area paging capabilities. Approximately 1500 pagers will be serviced. A schedule will be provided to do the service in increments. The total service will be complete by 30 Dec 98. The service will be performed off-site at the contractor's facility and the necessary frequency is M138.325. The offeror shall include procedures for retrieving the pagers from Hurlburt Field and returning pagers back to Hurlburt Field. FOB point is destination. Offers may be submitted on letterhead stationery or an SF1449 and at a minimum must show: (1) The name, address, and telephone number of the offeror; (2) description of your delivery procedures; (3) terms of any express warranty; (4) price and discount terms; (5) Representations and Certifications at FAR 52.212-3 and DFARS 252.212-7000; (6) past performance information, to include recent and relevant contracts for the same or similar equipment and other references (including contract numbers, points of contacts with telephone numbers and other relevant information); and (7) if the offer is not submitted on SF 1449, include a statement specifying the extent of agreement with all terms and conditions, and provisions included in the solicitation. The proposal must be signed by an official authorized to bind your organization. Offers that fail to furnish required representations and certifications, or reject the terms and conditions of the solicitation, may be excluded from consideration. FAR 52.212-2, Evaluation -- Commercial Items, is amended to read: "The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government anticipates award of a firm-fixed price purchase order. The following factors shall be used to evaluate offers: (1) past performance, (2) delivery procedures, and (3) price per pager.
