This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation RFQ 673-94-98. The standard industrial code is 3826 and the small business size is 500 employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-07. The James A. Haley Veterans' Hospital intends to negotiate a sole source procurement with Dynex Technologies, Inc., Chantilly, VA, for a MFX2 Microtiter Plate Fluorometer with Automatic Dual Dispenser for Reagents, and one 460 NM Emission Filter and one 612 NM Emission Filter. If submitting a quote on brand name equal, adequate technical information shall be submitted with the quote for the Government's complete evaluation. Failure to submit adequate technical information shall be grounds for rejection of quotations. Adequate technical information shall include but not be limited to: technical specifications, product availability, product picture, product features, etc. Quotations shall be submitted on an "all or none" basis. Partial quotations shall be rejected. All quotations shall state delivery time, F.O.B., F.O.B. point, shipping weight and cost if any. Offerors shall provide a complete set of provisions with proposal found at FAR 52.212-3, Offeror Representation and Certifications-Commercial Items. Provision and clauses found at 52.212-1, 52.212-4, 52.212-5 and 852.210-77 also apply.
