Texas Health Care System, Sam Rayburn Memorial Veterans Center, 1201
East Ninth Street, Bonham, TX wishes to procure Architect/Engineering
services for the construction of the "Upgrade Ambulatory Care Project"
at the Sam Rayburn Memorial Veterans Center, Bonham, TX. This project
shall upgrade deficiencies in our Managed Care and Primary Care
Service Facilities. Services shall include: development of construction
documents; design drawings and specifications; cost estimate; site
visits; as-built drawings; and CIH services. The overall scope of the
design and concept plan has been tentatively identified as; Audiology,
Dental, Eye and ENT Clinics, Executive Offices, Lab, Radiology, Lobby,
Medical Administrative Service, Mental Health, Nutrition, Nursing,
Parking, Police, PM&R, Prosthetics, Service Organizations, Social Work
and any other functions that may be identified during the design
process. The scope of the proposed project shall include; the main
entrance, non-assisted wheelchair access, renovations, fire protection,
medical gas (AOV), HVAC, electrical, structural, asbestos abatement,
signage, interior finishes, etc. The proposed contract is unrestricted
and competitive negotiations shall be used to award contract. Only A/E
firms that are headquarted within a one hundred and fifty (150) mile
radius of the Sam Rayburn VA, Bonham, TX are eligible to participate.
Requests from offerors located outside the geographic area shall not be
honored. The cost range of the construction project that shall follow
is between $1,000,000.00 and $2,000,000.00. All work under this
contract, exclusive of Government review time, shall be completed
within one hundred and twenty (120) days. The following evaluation
factors and their importance as expressed in percentages totally 100%,
shall be used in the source selection process to identify the most
highly qualified sources: (a) specialized experience and technical
competence in the type of work to be performed (20%); (b) ability to
design within construction budget cost limitations and prepare accurate
construction cost estimates (20%); (c) capability to perform work in
the required time (10%); (d) quality control methods used to insure
technical accuracy of work (10%); (e) professional qualifications of
staff and consultants proposed for the project (15%); (f) demonstrated
success in prescribing the use of recovered mateials; and achieving
waste reduction and energy efficiency in the design process (10%) (g)
experience and capability to use CAD (5%); (h) firms professional
reputation and standing (10%). CD's for the project shall be provided
on Auto CAD Version 14 along with one (1) set of Mylar reproducibilies.

Form 255 "Architect-Engineer and Related Services Questionnaire for
