Disease Control plans to enter into a purchase order/delivery order
with one contractor who can provide technical collaboration and
assistance on financial sustainability to 13 grantee communities funded
in support of their work with youth in delaying pregnancy and
childbearing. The tasks to be performed include: (1) completing a
standardized twelve-page cross site indicator (Government-furnished)
for each of the 13 communities; (2) provide consultation and guidance
on the development of annual sustainability plans based on an analysis
of the cross-site indicator; (3) develop a standardized format for
accumulating financial sustainability information from each community
in order to allow for regular point-in-time analysis of current status;
input information from each community into the standard format; (4)
provide consultation in design and implementation of a fund development
program, consultation to include individually-tailored presentations to
coalition members, boards of directors, local business partners, and
others based upon the needs identified by each community. Written

be required. Evaluation criteria include: Minimum 10 years experience
in working with teen and other adolescent programs to develop and
deliver services to youth; (2) Documentation of successful completion
of seminars in fund development to evidence that the applicant is
staying current with advances in the field of fund development and
financial sustainability; (3) minimum ten years of experience in
training and educating clients in successful implementation of their
own fund development programs including local health departments,
school systems, hospitals, and non-profit youth serving organizations;
(4) experience in successfully working with fund development
organizations; (5) experience in working in at least 4 regions of the
U.S.A. in fund development; (6) references from national non-profit
organizations that deal with youth that increased their funding as a
result of training and implementation of fund development strategies
