Lakes Healthcare System Acquisition Center intends to negotiate a sole
source contract with JMC Distribution of 1808 Halsted Street, Chicago
IL 60614 to provide and install a COSMED K4b2 Pulmonary Gas Exchange
System for the VA Hospital, Hines IL. The system will serve as a
nucleus for multidisciplinary clinical research focused on the
physiological and psychological aspects of pulmonary and cardiovascular
rehabilitation of our veterans. This is a combined
synopsis/solicitation for commercial items prepared in accordance with
the format in Far Subpart 12.6 as supplemented with additional
information included in this notice. SIC Code 3841. THIS ANNOUNCEMENT
CONSTITUTES THE ONLY SOLICITATION. A WRITTEN SOLICITATION WILL NOT BE
ISSUED. Solicitation documents and incorporated provisions and clauses
are those in effect through Federal Acquisition Circular 97-05. The
COSMED K4b2 system required will consist of the following: portable
unit with VO2 and CO2 Analyzers; Digital Turbine Flow Meter,
Non-Disposable Masks (3 each); with Harnesses (2 each); NI-MH
rechargeable batteries (3 each); battery charger unit, one year
standard commercial warranty (including parts and labor); PC Software,
Plug adapter for car, recharging head caps (2 each); carrying case,
user manual, Exercise Physiology Textbook and 1/2 Day on-site Training.
This procurement shall also include the following options: Digital
Telemetry Data Transmission System; Spirometry Software, Digital
Turbine Flowmeter and Calibration Syringe, 3-Liter. Training shall be
provided by the vendor and shall be scheduled with the CO upon delivery
and installation of the equipment. Payment will be made in arrears on
certified invoice upon final acceptance by the Government. This will be
a firm fixed price contract AND MUST INCLUDE ALL COSTS ASSOCIATED WITH
THE DELIVERY AND INSTALLATION of this equipment. Equipment shall be
shipped FOB is Destination to the VA Hospital Fifth & Roosevelt, Hines
IL 60141. The following incorporated provisions and clauses are the in
effect through Fac 97-05 and are applicable to this acquisition.

of the provision at 52.212-3, Offeror Representations and
Certifications-Commercial Items, with its offer. All equipment,
replacement parts, components, hardware, software and firmware
containing information technology delivered under this contract shall
be year 2000 compliant. Year 2000 compliance means that information
technology accurately processes date/time data (including, but not
limited to, calculating, comparing, and sequencing) from, into, and
between the twentieth and twenty-first centuries, and the years 1999
and 2000 and leap year calculations. The offeror is required, as part
of the offer, to provide documentary evidence that the equipment,
replacement parts, components, hardware, software and firmware offered
is year 2000 compliant. The proposed contract action is for the
purchase and installation of this equipment, which the Government
intends to solicit and negotiate with only one source under the
authority of FAR 6.302-1. Interested parties may identify their
interest and capabilities to respond to the requirement or submit
proposals. This notice of intent is not a request for competitive
proposals. A determination by the Government not to compete to this
proposed contract based upon responses to this notice is solely with
the discretion of the Government. Information received will normally be
considered solely for the purpose of determining whether to conduct a
competitive procurement. All responses must be submitted no later than
