Mick Dubuis 813-972-7593 This is a combined synopsis/solicitation for
commercial items prepared in accordance with the subparts in Federal
Acquisition Regulation 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; bids are being requested and a written solicitation
will not be issued. Solicitation number Request For Proposal (RFQ)
796-98-06 is assigned and shall be referenced on any proposals. This
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-04. The Standard
Industrial Classification Code is 3567 and the business size standard
is 500 employees. The James A. Haley Veterans' Hospital, 13000 Bruce B.
Downs Blvd, Tampa, FL 33612 has a requirement to purchase a
Pathological Incinerator for Elmendorf, AFB in Anchorage, Alaska. This
shall be a "turn key" procurement, with complete installation.
Requirement: PRIMARY CHAMBER -- 1 each, Pathological Waste Incinerator
(Primary Chamber with two (2) second retention time, chamber) complete
with burners, fans, control panel and chart recorders. Incinerators to
be factory wired and tested. Incinerator to meet the following
specifications: Rated capacity of 250 lbs/hr of type "4" pathological
waste, natural gas fuel with regulator, 2 each Burners 800,000 BTU each
with piping train, steel -- thick, Volume -- 107 cu.ft. minimum, Length
outside -- 8' 1", Width outside 7'6", Insulation -- Block 1900 F -- 2"
thick, Refractory-2700 F -- 4" thick, Full swing primary chamber
charging and clean out door, Heavy duty door bearing hinges, safety
switch on main and ash removal door, Positive cam door seal lock,
Refractory liquid retaining dams for pathological liquids, Combustion
air blowers and manifolds-dual ports, Auxiliary load door 24 x 24,
Primary chamber temperature control, secondary chamber temperature
control, primary chamber modulating air, secondary chamber modulating
air, stainless steel spark arrestor, view port in door, lifting eyes,
doors to have temp fiber glass door seal, positive cam door seal lock.
TWO SECOND RETENTION CHAMBERS (sits horizontal on top of primary
chamber) Two Burners 1,200,000 BTU each with piping train, Fuel-natural
gas with regulator, steel -- " thick, volume -- 107 Cu. Ft. minimum,
Length outside 8' 1", width outside 7' 6", Insulation-block 1900 F --
2" thick, Refractory -2900 F-4" thick, Height (floor to flange for
stack connection) -- 14', Two second retention unit must operate at
1800 F, Lifting eyes, ACS Post air injector Model AP-50 includes
-(combustion air blowers and manifold, air pre-heat chamber, modulating
combustion air supply), three (3) each 6' long refractory lined stacks
sections 12 gauge steel -- refractory 2300 F 3" thick lifting eyes,
Tie in stack section with cupola cap or in line dumper for future
envitech wet scrubber, roof penetration kit, automatic loader
hydraulically actuated, controls to interlock with the incinerator,
automatic ram face quench water spray, factory plumed wired painted and
tested, guillotine charging door, one (1) cubic yard hopper, full
extension charging ram. CONTROL PANEL -- enclosure NEMIA 12 UL
approved, PLC controllers based controlled -- automatically control and
monitor the process, primary and secondary temperature controllers,
lights for all burners and blower function, switch on-off, timer total
run time, timer 0 to 60 min, timer 0 to 6 hours, chart recorder for 2
second retention chamber record operating time and operating
temperature. INCINERTOR TO MEET CURRENT E.P.A. EMISSIONS STANDARDS,
other features -- safety screens-all fans, operating and maintenance
manuals (two complete sets, to include mfg technical literature,
operators manuals, service/calibration manuals and illustrated parts
breakdown), all exterior surfaces shall be prepared and prime coated,
plus a finished coat of industrial enamel. Contractor shall provide
installation review, system start up, refractory lining curing,
incinerator training (3 operators), incinerator operational testing
include processing/cycle. This contract shall be bid FOB destination
Anchorage. Price shall be all inclusive. Proposals shall be submitted
on an "all or none basis." Partial proposals shall be rejected. All
potential offerors shall be available to perform work immediately.
Offerors shall provide a complete set of provisions with proposal found
at FAR 52.212-3, Offeror Representation and Certifications-Commercial
Items. Provision and clauses found at 52.212-1, 52.212-4, 52.212-5 and
852.210-77 also apply. Proposals shall be submitted to the address

Mick Dubuis (90C). The government shall not be held responsible for
quality of fax transmittals. The solicitation number (RFQ number)
