The Operational Contracting Division intends to award a purchase order for Laboratory Testing; Pap Smear, Drug Screen, Forensic Blood Alcohol Testing. This acquisition is 100% set aside for small business. This is a combined synopsis/request for quotes. This announcement constitutes the only solicitation; proposals are being requested and a separate solicitation shall not be issued. This solicitation number, F29650-98-Q9147, is being issued as a request for quotes. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-4. The objective is to receive quotations for laboratory testing of Pap Sears, Drug Screening, and Blood Forensic Alcohol Testing. Contractor shall accomplish the following: Contractor shall provide all labor, parts, tools, and transportation necessary to provide any specialized collection tubes and swabs, to include shipping. Arrange for packing and pickup at least once daily. Contractor must be available on an "on call" basis for same day pickup and shipment of specimens on weekends and holidays. Contractor shall participate in quality control measures. Contract manager shall be available in person or by telephone during normal duty hours (0800-1600 MST). Contractor shall report the results of the tests within 24 hours of test completion via teleprinter. Results shall be received within 10 days of slide receipt. Performance will be between 1 Oct 98 and 30 Sep 99. New Mexico Gross Receipts Tax is required to be included in the offer. The Government will evaluate quotes in response to the solicitation without discussion and will award a purchase order to the responsible bidder whose bid, conforming to the solicitation, is most advantageous to the Government considering only price. Offeror's are reminded to include a completed copy of the provision at 52.212-3, Offeror Representations and Certification-Commercial Items, with their offer.
