commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; proposals
are being requested and a written solicitation will not be issued. The
solicitation number is TIRSE-99-Q-00002 and is being issued as a
request for quotation. The solicitation document and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-05. This requirement is set-aside for small businesses for
which the Standard Industrial Classification (SIC) Code is 8011. A
Medical Doctor is needed to act as Medical Officer for the IRS in two
locations in Covington, KY for the period 10/1/98 through 9/30/99, with
one year renewal option. Line item 001, provide a medical doctor to
serve as the medical officer for the internal revenue service (IRS)
located in Covington, KY for the period October 1,1998, through
September 30, 1999. The primary locations will be the Gateway Center
Health unit (GWC), 333 Scott Street, Covington, Kentucky and the
Cincinnati Service Center health unit (CSC) located at 200 West Fourth
street, Covington, Ky. These are the main IRS health facilities for
the Cincinnati Service Center. Services may be requested for other IRS
off-sites within the Counties of Boone, Kenton and Campbell in the
Northern Kentucky area on an as-needed basis. The service will be for
four hours each week at the GWC and four hours on the second and fourth
week at CSC. Except when mutually agreed upon by the contractor and
COTR, services will be performed in accordance with the following
monthly schedule: 8:30am -- 12:30pm: 1:00pm -- 5:00pm: 1st Wednesday

Wednesday Gateway Center 4th Wednesday Gateway Center Service Center
5th Wednesday Gateway Center The duties of the Medical Officer are as
follows: Provide physical assessments as requested by ordering
officials;The doctor shall provide emergency treatment of various
on-the-job injuries and illnesses and make referrals to hospitals or
outside physicians where necessary. The doctor shall initiate emergency
ambulance services when required; The doctor shall provide medical
emergency advice and treatment for non-IRS employees (visitors) and
children of the treasure house child care center. medical emergency

the treasure house child care center is required only while the
physician is on duty at the Internal Revenue Service. emergency
treatment for children of the Treasure House Child Care Center is
limited to treatment for potentially life threatening situations
pending arrival of an ambulance or other response team; The doctor
shall provide individual counseling to employee regarding matters of
personal, physical and mental or emotional health concerns. the doctor
shall refer employees to the IRS assistance program and or outside
sources where appropriate; The doctor shall consult with IRS management
for possible courses of action concerning difficult performance
problems that are health related; The doctor shall be available to IRS
management by telephone, limited to 30 calls per month, with no charge
to the Government. In most cases, each phone call will not exceed 15
minutes; The doctor shall oversee mass screening programs for IRS
employees to detect visual defects, glaucoma, diabetes, blood pressure
problems, hearing conservation, cancer tuberculin test, etc. positive
findings will be referred to the employees private physicians,
immunizations against such diseases such as tetanus, influenza, etc.,
shall be provided; The doctor shall provide special medical treatment
to employees upon written orders from the employees private physicians;
The doctor shall work with management personnel and safety officials in
developing and maintaining a full range of employee health programs and
in providing a healthful working environment; The doctor shall
evaluatechronic sick leave cases and consult with management on serious
health problems. It may be necessary at times for the doctor to consult
with outside medical resources; The doctor shall be responsible for
proper maintenance and confidentiality of records; The doctor shall
provide analytical and progress reports on an as needed basis; The
doctor shall advise management and assist with employee requests for
reasonable accommodations and disability requirements in accordance
with the Federal Personnel Manual (FPM), chapter 339, Medical
Qualification Determinations, 5 Code of Federal Regulations (CFR) part
339, other guidance from the Office of Personnel Management (OPM),
Equal Employment Opportunity Commission (EEOC), or other higher
authority, as well as IRS and Service Center Regulations. The doctor
may conduct the actual physical exam, or review the results of employee
examinations by personal physicians to determine if the findings in the
medical evaluation results are justified and the conclusions reached
are consistent with accepted medical practice. The doctor may be
required to review position descriptions, environmental factors, and
determine the value and nature of a specific accommodations in relation
to a medical condition; The medical doctor will arrange and assume
responsibility for a substitute doctor to perform contracted duties in
the event of absence; The doctor shall provide other related services
as necessary; The Doctor shall maintain the appropriate Licensing and
Insurance to practice within the State of Kentucky. Provisions
