407/380/4306 17. This is a combined synopsis/solicitation for
commercial items prepared in accordance with the format in Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; quotations
are being requested and a written solicitation will not be issued.
Solicitation N61339-98-Q-1076 is issued as a request for quotation and
incorporates the provisions and clauses in effect through Federal
Acquisition Circular 97-07. This solicitation is set-aside for small
business. The SIC Code for this solicitation is 7378 and the small
business annual receipts of $18M. The following service is required:
CLIN 0001, Annual Maintenance Service for 165 Hewlett Packard Laserjet
Printers as per the following statement of work. Statement of Work
(SOW). Part 1 GENERAL. 1.1 GENERAL DESCRIPTION. Scope. This statement
covers the general requirements for contractor furnished technical
services and materials to be used for performing essential maintenance
on Government-owned, nonstandard equipment. Essential maintenance can
include preventative maintenance, essential repair in the event of
equipment breakdown, and/or alignment and calibration. This statement
does not include requirements for modification of equipment. 2.
Contractor Services: The contractor shall provide technical services,
Monday through Friday (0800-1600 hrs), as required to accomplish the
following. 2.1 Requests for Service: a. Requests for maintenance
service under the terms of the contract will be placed by a telephone
call on an "as required" basis by the DFAS-Orlando. Administrative
Support Division (DFAS-Orlando). Contractor shall not accept, or
respond, to requests for service which are placed by anyone other than
the above designated representatives or the Contracting Officer. b.
Provide necessary repair. Normal response following a request for
repair shall be within eight (8) business hours after the contractor is
notified that equipment is inoperative or otherwise in need of repair.
c. Provide all labor, travel and parts determined necessary to
maintain the equipment in good operating condition during the specified
period of coverage. Replacement parts shall be new or equivalent. d.
Contractor services shall be required to be performed at the physical
location of the equipment, DFAS-Orlando, except that provisions should
be made for factory repair of individual components, assemblies, or
subassemblies of the end item equipment, if the discrepancies are such
that local repair of the applicable item cannot be accomplished.
Factory repairs will be accomplished at no additional costs to the
government. 2.2. Inspection: The equipment shall be inspected, bench
checked or tested to detect malfunctions, or other unsatisfactory
conditions and to determine the nature, extend and feasibility of
repairs required to return the equipment to serviceable condition. 2.3
Preventative Maintenance. Preventative maintenance, as used herein, is
the normal inspection, up keep and preservation of equipment which may
be required from time to time to verify or restore complete
serviceabililty of equipment which has been subjected to usage, wear
and tear, or deterioration caused by environmental conditions.
Preventative maintenance includes but is not limited to cleaning and
servicing, alignment and adjustment performance checks, the detection
and repair of minor defects which, if allowed to accumulate, could
develop into major defect which would render the equipment serviceable.
Preventative maintenance will be performed at least two (2) times per
year. 2.4 Repair: Equipment shall be repaired, as necessary to correct
each malfunction, deficiency, or other unsatisfactory condition
discovered during inspection, bench check, or test. Repair includes: a.
Cleaning and servicing. b. Removal, repair or replacement, and
reinstallation of parts or material determined to be defective. All
assemblies, subassemblies and parts determined to be serviceable during
inspection or bench check shall continue to be used. All parts
determined to be defective shall be repaired or replaced. c.
Adjustments. d. Alignment and calibration if the nature of the repairs
is such that the alignment and/or calibration of the equipment or
individual components thereof is affected (See Paragraph 2.5). e. Any
other work necessary to repair the equipment. f. Bench checks,
functional and/or performance tests required to determine that the
equipment has been restored to a serviceable condition and is capable
of accomplishing its intended purpose. g. Repairs shall be accomplished
in accordance with applicable maintenance manuals or, in the absence of
such manuals, in accordance with good commerical practices. 2.5
Alignment and Calibration: The contractor shall align and calibrate the
equipment, as necessary, to insure that its operational performance
characteristics are equal to or better than the performance
characteristics accepted as satisfactory by the using organization
during acceptance tests. Alignment and calibration shall be
accomplished in accordance with applicable alignment and calibration
data for the equipment or, in the absence of such data, in accordance
with good commercial practices. 3. Performance: The contractor shall
assure that the performance of the equipment after services rendered
shall be such that the equpment meets the requirements and performs the
function for which it was originally designed or modified and that it
is in a completely serviceable condition. 4. Government furnished
parts: Government furnished parts shall be used when available and
readily obtainable from the using organization. 5. Contractor furnished
parts: Contractor shall furnish all parts not immediately available
from the using organization. Parts furnished by the contractor shall be
limited to those parts used by the contractor during repair of the
equipment as direct replacements for defective parts and shall be of
equal or better quality, and directly interchangeable with the replaced
parts. Contractor shall not furnish any additional parts for future use
(spare parts). 6. Hand tools: The contractor shall furnish his own hand
tools. Test equipment: Normally, the contractor shall furnish his own
test equipment. However, when government-owned test equipment is
available at the equipment location it may, subject to the using
organization, be made available for contractor use. 7. Period of
Performance: 98OCT01 -- 99SEP30. END OF STATEMENT OF WORK. The
following FAR provisions and clauses apply to this acquisition:
52.212-1, 52.212-2, 52.212-4, and 52.212-5. The following evaluation
criteria shall be used as addenda to FAR 52.212-2(a): (1) Technical
capability of the item offered to meet the Government requirement; (2)
price; and (3) past performance. The following subparagraphs of FAR
52.212-5(b), and DFARS addenda thereto, apply: (1), (3), (6), (7), (8)
and (9); 252.225-7001 and 252.225-7036. A completed copy of FAR
52.212-3 and the following DFARS addenda, must be included with each
quote: 252.225-7000 and 252.225-7035. See Note(s) 1 and 12. Quotes are
