65 -- UPGRADE X-RAY TRAINING EQUIPMENT AND TRAINING MANUALS SOL 541-263-98 DUE 092898 POC Sherry A Farley (440) 838-6060 FAX (440) 838-6052 This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; Request For Quotations (RFQ) are being requested and a written solicitation will not be issued. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Number 97-06. This procurement is set-aside for small businesses and the SIC Code is 7699 and the small business size standard is $5.09 million. The Little Rock Education Center, (LREC), Employee Education System (EES) plans to upgrade three rooms of X-ray training equipment. SCOPE: The work to be performed is to upgrade three rooms of X-ray training equipment and to add a (PACS) Network Workstation in Building 58 of the Little Rock Education Center, EES at North Little Rock, AR. All the equipment will have DICOM compatibility. Customized documentation and lab manuals for each system will be provided and the current training documentation used for x-ray training at the center upgraded. a. Background: The current x-ray equipment used for training at the LREC is analog and is now obsolete. Digital equipment is now needed to upgrade the training labs. b. Objective: With this upgrade of the equipment, the LREC will be able to offer digital x-ray training for VA personnel. This will bring the training standards up to state-of-the-art and allow additional courses to be offered. APPLICABLE DOCUMENTS: Contractor will provide all manufacturer manuals that will be used to customize documentation and lab manuals. SPECIFIC TASKS: Room 1, upgrade the current General Electric Radiographic/Fluroscopic equipment to include: Item 0001- one(1) new 9" Tri-mode Image Intensifier; Item 0002 -- one (1) new Rodenstock CCD Camera/Lens; Item 0003 -- one (1) new, Digistar Digital Imaging System; Item 0004 -- one (1) new 17" television monitor. Room 2 -- upgrade the current Philips Radiographic/Fluroscopie equipment with reconditioned parts and provide: Item 5 -- one (1) new CCD Camera; Item 0006 -- one (1) new Digistar Digital Imaging unit; Item 0007 -- one (1) new 17" television monitor. Room 3 -- upgrade the current Siemens Urology equipment with Item 0008 -- reconditioned Polyphos 50 generator; Item 0009 -- reconditioned Uroskop table; Item 0009 -- one (1) new CCD Camera; Item 0010 -- one (1) new Digistar Digital Imaging unit; Item 11 -- one (1) new 17" television monitor. Addition: PACA Network Workstation to work with above equipment; The old equipment will be removed and the listed equipment installed and interfaced as required; The contractor shall furnish all parts and labor; Complete Training Documentation shall be provided to match each room of equipment with the courses offered at the Little Rock Education Center. This shall be coordinated with the COTR. Complete Lab Manuals shall be provided for each room to match the courses offeredat the Little Rock Center. An electronic copy and a camera ready master shall be provided for each set. The content and format shall be approved by the COTR. The materials must be suitable for standard university courses since the University of Arkansas will give college credit for these courses. Three years of PM and maintenance shall be included with the upgrade for each room. This shall be for all the equipment in each room and will be monitored by the COTR. The contractor shall provide appropriate documentation for all PM and maintenance. The maintenance program shall assure that the equipment is in proper condition for all classes with a two-week notice provided to the contractor prior to each class. Documentation shall be provided for the PACS Network Workstation that can be used in the training classes. PERIOD OF PERFORMANCE: 120 days after contract is awarded. PROPRIETARY INFORMATION STATEMENT: The contractor shall be required to negotiate agreements with commercial system vendors relating to non-disclosure of vendor-proprietary information for training manuals. LEVEL OF EFFORT: The contractor shall be required to remove any existing equipment that is replaced and make the complete installation and interface of all replacement equipment. The contractor shall develop and produce an electronic and a camera ready copy of all documentation and manuals. The contractor shall be responsible for procuring all the equipment, parts and manuals required to accomplish the work. GOVERNMENT FURNISHED EQUIPMENT: The government will furnish copies of current training materials and tools that are currently available in the Little Rock Education Center. The government will not be responsible for providing any other tools or equipment. PLACE OF PERFORMANCE: X-ray labs, Building 58, Little Rock Education Center, VA Medical Center, 2200 Fort Roots Drive, North Little Rock, AR 72114. WARRANTY: OFFER SHALL PROVIDE COMMER CIAL WARRANTY IN ACCORDANCE WITH MANUFACUTRE SPECIFICATION. F.O.B. Destination: Delivery shall be F.O.B. destination. Offerors responding to this announcement shall submit a technical proposal and cost proposal. Identify at least three sources for past performance verification. Contractor proposal shall also contain the information required in provisions 52.212-1, a completed copy of 52.212-3 and the certification required in 852.219-70.
