Edwards, Contracting Officer (414) 902-5418 The Department of Veterans Affairs (VA), Great Lakes Acquisition Center, Milwaukee, WI, is soliciting responsible sources interested in providing professional services, of a board certified Orthopedic(s) and an Ear, Nose and Throat specialist(s), at the VA Medical Center, 3001 Greenbay Road, North Chicago, IL 60064. The contractor(s) will be responsible for providing examinations during Orthopedic and Ear Nost and Throat clinics, for the period of one year from date of award. This requirement has two additional one-year performance periods at the option of the government. Examinations are based on Medicare Codes for the State of Illinois, 1998 Medicare Rates (locality 15) schedule. The services include performing examinations and making medical assessments and furnishing opinions, which are supported by their findings. Physicians will also provide a dictated summary and report on each beneficiary seen. The physicians will confer with VA staff including the Clinic Program Manager in order to initiate referrals or consults. The proposal must include curriculum vitae, state medical school license(s); medical school certificates, if applicable, etc. See solicitation for further details. The SIC Code for this procurement is 8099 and is a full and open competition, fixed-price contract. The Small Business Standard is $5.0 million.
