This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (A) The solicitation number is F41612-99-T0057 and this is a Request for Quote. (B) This solicitation incorporates the provisions and clauses in effect through Federal Acquisition Circular 97-08. (C) This is sole-source set-aside for PROTOCOL, Inc. Standard Industrial Code is 3823. Size Standard is 500 employees. (D) Line Item 0001 Monitoring System, Propaq 102/EL, includes: one (1) cuff kit, expansion module 3-channel printer, pulse oximeter and one (1) DS100A durasensor (CAT#900-0021-00). Cuff kit includes: small and large arm and thigh cuffs (CAT#008-0324-00). Also includes two (2) operation procedures per unit, two (2) complete service manuals with fully legible schematics per unit (same as company service representatives' manuals), two (2) copies of operating and service software per unit. The quantity is four (4). The required delivery date for these items is 1 February 1999. Quotes must include price for FOB-Destination and/or Origin. Contractor is required to complete Federal Acquisition Regulation Clause 52.212-3, Representations and Certifications Commercial Items, and present the representations and certifications with their cost proposal.
