synopsis/solicitation for commercial items prepared in accordance with
the format in FAR Subpart 12.6, as supplemented, with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. (A) The solicitation number is
F41612-99-T0055 and this is a Request for Quote. (B) This solicitation
incorporates the provisions and clauses in effect through Federal
Acquisition Circular 97-08. (C) This is sole-source set-aside for
Medical Energy, Inc. Standard Industrial Code is 3841. Size Standard is
500 employees. (D) Line Item 0001 YAG Laser: Surgical laser system,
1064NM, 40W, 110V single-phase, includes: two (2) pair clear lens
safety eyewear, two (2) safety signs, one (1) operator's manual with
start-up operation procedure per unit, one (1) complete service manual
with fully legible schematics per unit (same as company service
representatives' manuals), full in-house technical training for three
(3) biomedical technicians (includes airfare, hotel, meals in Pensacola
FL at any date and time agreed upon by 384th Clinical Engineering and
Medical Energy, Inc.). The quantity is two (2). The required delivery
date for these items is 1 February 1999. Quotes must include price for
FOB-Destination and/or Origin. Contractor is required to complete
Federal Acquisition Regulation Clause 52.212-3, Representations and
Certifications Commercial Items, and present the representations and
certifications with their cost proposal. Facsimile proposals are

Mailed/faxed cost proposals must be received by close of business (4:00
CST), 4 December 1998. Mailed proposals can be mailed to 82d

ID numbers. Potential contractors are reminded that in order to be
eligible for award of a contract they must be registered with the

Instructions to Offerors Commercial, applies to this acquisition
without addenda. Award will be based on the contractor quote determined
to be the best value to the government concerning price and delivery
