is a combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; proposals are being requested and a
written solicitation will not be issued. This solicitation
(F26600-99-Q-0212) is issued as a request for quotation. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-05. The SIC for this
solicitation is 1752. The small business size standard is $7.0 million
in annual gross revenues for the three previous fiscal years. Item
description as follows: Nonpersonal service to provide all labor,
equipment, tools, materials, supervision, and all other necessary items
to remove old carpet and cove base, and install contractor-furnished
carpet and cove base at building 200, on Nellis AFB, Las Vegas, NV.
89191. Carpet to be installed on the 1st floor hallways only and
includes 450 SY of carpet tiles of Tunisia (2557) or equal, with 12"
border of Capuccino (38007 Bedouin) or equal and 4" wall border, bound
edge, of the same. A site visit will be held on 22 Dec, at 2:00 p.m.
PST. You must notify us by fax (702-652-5405) with the names and
numbers of attendees by the close of business 21 Dec 98. Offers are due
no later than close of business 4:30PST (local time) on 28 Dec 98.
Award will be made on or about 31 Dec 98. All responsible sources may
submit a quote, which if timely received will be considered by this
agency. Defense Priorities and Allocations System (DPAS) assigned
rating: DO S10. The provision at FAR 52.212-1, Instruction to
Offerors-Commercial, applies to this solicitation. The clause at FAR
52.212-2; Evaluation of Commercial Items, applies to this solicitation.
Award will be made to the responsive, responsible offeror whose offer
represents the best value to the Government based on technical
capability, price and past performance. Performance will commence on or
about 11 Jan 99. This will be issued as a firm fixed price purchase
order. The Gov. Rep. is the primary individual designated to
participate in the administration of this contract to insure that the
contractor complies with its technical requirements, including
inspection and acceptance of the service for the government at the
performance site. For this service agreement the Gov. Rep. is MSgt

Inspection of Services Fixed Price (Aug 1996) applies to this
solicitation and subsequent award. The contractor's invoice shall show:
Contract/Purchase Order Number, Billing Period, Amount, and Name of
Service. Invoices shall be sent to: DFAS-SB Operating Location, 1111 E.
Mill Street, San Bernardino, CA 92408. Permission for contractor
personnel and vehicles to enter the installation is subject to the
approval of the Security Police. Request for vehicle & ID passes will
be submitted to 99 CONS/LGCV. It must contain name, social security
number,and two photo ID's of the individual(s) and
make/model/year/color and license number of vehicle. MINIMUM
QUALIFICATION REQUIREMENTS ARE: Offerors bidding on this RFQ are
required to submit along with their quote a maximum of one page on the
following: a list of past contracts performed related to this RFQ
within the past year. Include description, amount, names and telephone
number for references. Work shall be performed Monday-Friday
(excluding federal holidays) from 0730-1630. Offerors must include with
their price proposal a completed copy of the provision at 52.212-3,
Offeror Representation and Certification Commercial Items, offerors who
fail to submit a completed Reps. and Certs. may be considered
non-responsive. The clause at 52.212-4, Contract Terms and Conditions
Commercial Items, applies to this acquisition. The clause at FAR
52.212-5, Contract Terms and Conditions Required to Implement Statutes
or Executive Orders Commercial Items, applies to this solicitation.
