Center, Cleveland Ohio proposes to enter into a contract for the
services of an instuctor to teach a comprehensive training program with
the objective of training Biomedical Engineering Technicians to repair
and maintain radiological equipment. There are a total of six (6)
courses. The first five courses are X-Ray Training I, II, III, IV and
V and each will be two (2) weeks duration. The last course is
Ultrasound Systems Maintenance which is also two (2) weeks duration.
There will be approximately four (4) weeks between courses for on the
job assignments. This is a Request For Quotations (RFQ). This
acquisition is set aside for small business for which the Standard
Industrial code is 8748 and the small business size is $5.0 million. It
is anticipated that a firm fixed price contract will be awarded. This
is a combined synopsis/solicitation for commercial items prepared in
accordance with the format in FAR subpart 12.6 as suplemented with
additional information included in this notice. This annoucement
constitutes the only solicitation; quotations are being reqested and a
written solicitation will not be issued. The Contractor shall provide
the services/supplies identified as follows: 1) Contractor shall
provide instructional staff that will consist of one (1) instuctor for
each module. 2) The Contractor shall document the company's experience
in providing instuction and management of a hands-on Medical
Radioligical Repair and Maintenace training program. A minimum of two
(2) years experience in this field is required. 3)The Contractor shall
provide resumes of instructors who will be involved in the training.
Each instructor must have taught hands-on maintenance of medical (not
dental) x-ray systems for at least five (5) years. 4) The Contractor
shall provide a list of previous contracts of similar medical
radiological maintenance training programs including company name,
address, telephone number and name of person who can evaluate the
quality of the program presented. An unsatisfactory evaluation could
disqualify a bidder. 5) The Contractor shall provide complete student
workbooks and lab manuals. Approval of Contracting Officers Technical
Representative (COTR) EES Little Rock Center, Little Rock AK is
required prior to being used in classes. Approval can be made as
materials are developed. Prefer approval prior to starting each module,
however additional material can be added with twenty-four hour (24)
approval. 6) Test and quizzes shall be used in program. Final
examination for Module III must test to determine whether or not
student has achieved capability to troubleshoot imaging problems with
seventy percent (70%) accuracy. All materials developed/produced by the
Contractor for this program will become the property of the Department
of Veteran's Affairs including copyrights to the workbooks, lab
manuals, tests and quizzes. 7) The Contractor shall provide one (1)
working Acuson, HP and one (1) ATL Ultrasound unit and all test
equipment for this training. 8) The Contractor shall provide
finalgrades to the COTR EES Little Rock Center within seventy-two (72)
hours after the final exam for each class. Narrative evaluations on
each participant for each course will be furnished within one (1) week
after completion of the course. All responsible sources may submit an
