contractor to provide labor, materials, and supervision necessary to
remove 165 existing fluorescent lighting fixtures and furnish and
install 50 new 175 watts luminaries LW 17 MH Q AC12 (OR EQUAL) in
accordance with the specifications of work. This is a combined
synopsis/solicitation for commercial items prepared in accordance with
Subpart 12.6, as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation; a
written solicitation will not be issued. This acquisition will be
subject to Federal Acquisition Regulation (Far) Part 13, Subpart 13.5
regarding the application of special simplified acquisition procedures.
This is 100% small business set-a-side, issued as a request for
quotation (RFQ), number SSA-RFQ-99-3751. Quotations submitted from
other than small business sources will be rejected. All responsible
small business sources may submit a quotation, which will be considered
by the Agency. The standard industrial classifications (SIC) code for
this acquisition is 2599. The small business size standard is 500
employees. This request for quotation and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 97-09.
Written quotations in response to this RFQ shall be submitted by
February 2, 1999, 4: 00 p.m. Eastern Standard Time, addressed to the
Social Security Administration Attn: Sharon Hall, 1710 Gwynn Oak
Avenue, Baltimore, Maryland 21207, or faxed to (410) 966-9310. Should

SPECIFICATION OF WORK: The contractor shall remove and dispose of
approximately 165 existing fluorescent light fixtures and lamps. The
contractor will also be responsible for removing the tubes and ballasts
from these fixtures and packaging them in a way that is suitable for
the recycling contractor. The contractor shall then furnish and install
50 new Metal Halide fixture to include all necessary wiring hardware
etc., using existing circuits and conduits where possible. Lights shall
be pendant mount type, 175 watt Metal Halide Multi Tap Luminaries or
equal including bulb. The contractor shall remove all conduits and
mounting brackets associated with the fluorescent fixtures that are not
used in the installation of the Metal Halide light System.
RESPONSIBILITY: The contractor shall be responsible for all necessary
work permits, safety barriers, disposal of existing fixtures and any
construction debris. The contractor shall package all tubes and
ballasts that are removed from the existing fixtures. At no time will
it be permissible to throw any trash or debris in Government trashcans
or dumpsters. All work shall be performed before 6:00 a.m. and after
3:00 p.m. on weekdays or on weekends from 6:00 a.m. to 6:00 p.m. and
shall not hamper the normal building operation. The building must be
fully operational between the hours of 6:00 a.m. and 3:00 p.m. All work
shall be completed 60 days from date of award. INSPECTION: Prior to the
start of work the contractor shall inspect all locations where work is
to be performed and notify the Project Officer in writing of
conditions detrimental to the proper and timely completion of the work.
Do not proceed with the work until all unsatisfactory conditions have
been corrected in a manner acceptable to the Project Officer and in
accordance with the contract. SYSTEM WARRANTY: Contractor shall provide
a two-year full parts and labor warranty for all work performed. After
completion and acceptance of this requirement, the contractor shall
provide to the Project Officer a list of warranted items/services.
COMPLETION: Upon completion of all work as outlined above, the
contractor shall remove all materials, equipment and debris that was
removed and/or replaced. SITE INSPECTION: A site inspection will be
held on Friday, January 29, 1999 at 10:00 a.m. at the Metro West
Building, 300 North Greene Street, Baltimore, Maryland 21201. Please
meet in the lobby of the building. The following provision at FAR
52.212-1, Instruction to Offerors-Commercial Items, (tailored) apply
this acquisition: (b) (1), (b) (2), (b) (3), (b) (6), (b) (7), (b) (8),
(b) (9), and (b) (10). There are no addenda to these provisions. The
provision at FAR 52.212-2, Evaluation-Commercial Items (tailored) apply
to this acquisition: (a) and (c). The Government will award a contract
from this request for quotation to the responsible offeror whose offer
conforming to the quotation will be most advantageous to the Government
price and other factors considered. The following factors shall be used
to evaluate offers: Price and Past Performance. These factors are equal
in importance. To determine an offeror's acceptability relative to past
performance and as discussed in FAR Clause 52.212-1 (b) (10);, offerors
shall provide 3 references of customers for whom a job similar to this
solicitation was performed. Offerors are required to verify the names
of contacts, addresses and telephone numbers of all references. The
Government will only make two attempts to contact references submitted.
Offerors are advised to include a completed copy of the FAR Part
52.212-3, Offeror Representation and Certifications-Commercial Items
with their offer. The provision at FAR Part 52.212-4, Contract Terms
and Conditions-Commercial Items apply to this acquisition. Provision at
FAR Part 52.212-5, Contract Terms and Executive Orders-Commercial
Items, apply to this acquisition, specially: (a), (b) (7), (b) (8), (b)
