VA Healthcare Network Upstate New York-Albany, NY is seeking to contract with local established hospitals, ambulatory care facilities and/group practices in Plattsburgh community who have the qualifications, experience, and expertise in providing quality medical care services. Services shall be provided to veteran beneficiaries residing in Plattsburgh and surrounding communities within the Facility's Primary Service Area. The Offeror/Basic Provider shall provide complete Primary care services in a private hospital or clinic environment. Primary care includes longitudinal outpatient medical care for the purpose of prevention and detection of disease and subsequent management of medical conditions in eligible veterans. The care will be provided by primary care providers who may be general practitioners or general internists or Nurse Practitioners. The care provided should be continuous, accessable, and coordinated.
