440.838.6091 This is a combined synopsis/solicitation for commercial
supplies/services prepared in accordance with the format in FAR Subpart
12.6, as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation; Request
for Quotation (RFQ) are being requested and a written solicitation will
not be issued. Solicitation documents and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 67.
This procurement is a 100% small business set-aside The Standard
Industrial Classification Code is 3842 and the small business size
standard is 500 employees. Item (1) 32 Channel CardioLab EP System, the
System shall include the following: ClabPro 800 System Controller to
include 233 MHz Pentium processor, 64MB of Ram memory, PC keyboard and
trackball, 2GB High Performance Hard Disk Drive, High Performance
Optical Drive II System; Amplifier Unit: Superior RF Filtering and 64
catheter input capability with 2 catheter input moduls, 12 Lead ECG, 4
pressure inputs, and 50' fiber optic cable. The amplifier unit shall
meet UL2601-1 safety standards for medical equipment and shall have
been independently tested by ETL testing laboratories; Monitors and
Printer: Two (2) 21" flat screen high resolution (1600 X 1280 color
monitors LaserJet with 600dpi capability, 12 page per minute, and full
page graphics, includes 2MB of memory; Software: Current version of
CardioLab EP Electrophysiology Software, Fax and Modem communication;
Accessories: Workstation mobile cart with swivel arms, 60" (153cm) Wide
X 34" (86cm) Height X 30" (77cm) Depth, 33.3K Baud modem, Spare 50'
fiber optic cable, spare set (2) catheter input modules, four (4) 650
MB optical disk cartridges, isolation safety transformers and
un-interruptible power source and manuals; Training and In-service:
Three (3) day installation and in-service, and two (2) day training at
Contractors facility for one (1) nurse and technician, which shall
include tuition, lodging and any airfare, and applications follow-up;
Warranty: Standard Commercial on all hardware and on all cables,
including catheter input modules, software maintenance updates for the
life of the equipment; Support: 24 hour toll free customer service,
remote modern diagnostics, next business day loaner equipment service
and on-site engineer service if deemed necessary, Stock Number
211-00102-00, Quantity 1 each; Item (2) 21" Flat Screen High Resolution
Auxiliary Monitor (1600 X 1280 for use as a slave to primary monitors,
Stock Number 211-00450-00, Quantity 1 each; Item (3) 50 foot video
cable db15 to 5BNC connectors used to connect a monitor to the computer
or workstation, Stock Number 301-00117-50, Quantity 1 each; Item (4) RF
Ablation Device Interface Module, includes the necessary hardware and
software for recording and displaying real-time information from the
following RF ablation units, Ablation Devices Supported, EP
Technologies, Inc., EPT-1000 Cardiac Ablation Controller, Medtronic
CardioRhythm Atakr RF Generator, Osypka HAT300 (RF Generator) and
HAT200 (RF Generator), Stock Number 211-00405-00, Quantity 1 each; Item
(5) Installation, Quantity 1 job, the VA will contact the Contractor as
to the date and time to begin installation. Contractor shall include in
the RFQ an equipment trade-in for a Quinton "EP Lab" for
electrophysiology system and Digital Cardiovascular Instruments,
"Clinical Simulator, Model EP-2. These items shall be delivered to the
Louis Stokes Cleveland, Department of Veterans Affairs Medical Center,
10701 East Boulevard, Cleveland, Ohio 44106, Attention: Warehouse. All
items shall be delivered within sixty (60) days after receipt of
purchase order. Delivery shall be F.O.B. Destination. The Contractor
shall warrant that each hardware, software, and firmware product
delivered under this solicitation, shall be able to accurately process
data from, into, and between the twentieth and twenty-first century,
to include, leap year calculations. The following provisions apply to

completed copy of provision 52.212-3, and the certificaiton required in
852.219-70. Quotations shall be received by March 8, 1999 at 4:30 p.m.
