This is an amendment to a synopsis that was originally published on
January 25, 1999. The purpose of the amended synopsis is to clarify and
provide further instruction regarding method of solicitation and
proposal submission. This requirement is herein solicited as a combined
synopsis/solicitation for commercial items prepared in accordance with
the format in Federal Acquisition Regulation (FAR) Subpart 12.6 as
supplemented with additional information included in this notice. This
announcement constitutes the only solicitation; proposals are being
requested and a written solicitation will not be issued. The
Solicitation Number is 1999N00047(N) and is issued as a Request for
Proposal (RFP). The Government will award a contract resulting from
this solicitation to the responsible offeror whose offer conforming to
the solicitation will be most advantageous to the Government, price
and other factors considered. The solicitation and incorporated
provisions and clauses are those in effect through Federal Acquisition
Circular 97-08. This procurement is solicited on an unrestricted
basis. The Standard Industrial Classification Code (SIC) is 3821, with
the small business size standard as no more than 500 employees. The
purpose of the procurement is to Replace Two Autoclaves at Building 15,
Centers for Disease Control and Prevention Clifton Road Campus,
Atlanta, Georgia. In addition to providing and installing the
replacement autoclaves, the contractor shall provide final hookup of
all utilities at the autoclave units and start the units to ensure
functionality. The second unit will be installed approximately six (6)
to ten (10) months after the first, due to the operational
requirements of the laboratory. Removal of existing autoclaves;
preparing the building to receive the new autoclaves; and installation
of utilities for connection of new autoclaves will be performed by the
Government. Copies of the specifications and the one-page drawing are
available on request from Bill Bancroft, FAX: (770) 488-2670. Last date
to request this package is March 29, 1999. The contractor shall provide
full warranty coverage for the system for a one-year period after
installation. Delivery shall be made within 60 days of contract award.
Delivery shall be FOB Destination to CDC, Building 15 Loading Dock,
1600 Clifton Rd., NE, Atlanta, Georgia 30333. Offerors shall use the
Standard Form 1449, "Solicitation/Contract/Order for Commercial Items"
form found at URL: http://www.arnet.gov/far/. Payment terms are 100%
upon completion of installation and demonstration of compliance with
specifications. The provision at FAR 52.212-1, Instructions to Offerors
-- Commercial Items is applicable without addenda. The provision at FAR
52.212-2, Evaluation -- Commercial Items is applicable. Technical merit
and past performance, when combined, are significantly more important
than cost or price. Evaluation of technical merit will be based on
technical capability of the item offered to meet the Government
requirements. Specifictechnical information to be submitted shall
include: (1) Reference information regarding experience of firm with
installation of five (5) similar projects within the last three years.
Include client name, contact person, telephone number, project
description, contract award amount, amount of nature of change orders
and adherence to the schedule. (2) Reference information regarding
experience of firm providing maintenance and/or service on five (5)
similar projects in the past three years. List type equipment and type
maintenance performed, response time from receipt of client request to
completion of service call. Include contact information as above. (3)
Reference information on five projects of similar nature regarding the
performance of the installation crew employed. State whether
installers were your firm's employees or subcontractor, and state
installation experience of each crew member proposed to work on this
project. Award of this procurement will be based on overall best value
to the Government, with consideration given to the factors of proposed
technical merits, price and past performance. For selection purposes,
technical, price, and past performance are essentially equal in
importance. It is critical that offerors provide adequate detail to
permit evaluation of their offer. Offerors must include a completed
copy of the provision at FAR 52.212-3, Offeror Representations and
Certifications -- Commercial Items. The clause at 52.212-4, Contract
Terms and Conditions -- Commercial Items is applicable. The following
additional terms and conditions are included by addenda: FAR 52.214-34,
Submission of Offers in the English Language (APR 1991); FAR 52.214-35,
Submission of Offers in U.S. Currency (APR 1991); FAR 52.246-17,
Warranty of Supplies of a Complex Nature (APR 1984). The clause at FAR
52.212-5, Contract Terms and Conditions Required to Implement Statutes
or Executive Orders -- Commercial Items applies to this action. The
