D.C. 20201 R -- MODIFICATION TO CONTRACT FOR EVALUATION OF THE ALZHEIMER
DEMONSTRATION GRANT TO STATES PROGRAM. POC Point of Contact: Doris
Pearson, Contract Specialist, 202-690-6549 The Department of Health and
Human Services intends to negotiate with the University of Kansas,
Gerontology Center, on a sole source basis under the authority of FAR
6.302-1 to modify the "Evaluation of the Alzheimer Demonstration Grant
to States Program," contract. This contract was awarded to the
University of Kansas on a noncompetitive basis in August 1998. The
purpose of the contract is to (1) support the continuation of data
collection and monitor for accuracy, completeness and quality, the
collection of all program data from the 15 Alzheimer's Demonstration
Grants to States Program State Grantees; (2) to utilize the data in (1)
above, and other previously collected data, perform an analysis and
prepare a report for the 1997 grant year; (3) receive technical
assistance, coordination of data collection activities and monitoring
for accuracy, completeness and quality, for the Managed Care
Initiative; (4) receive consultation services for the HRSA/BPHC
evaluation strategy; and (5) receive consultation services to the
Project. The purpose of this modification is to provide continued
support of the data collection and monitoring for accuracy,
completeness and quality, the collection of all program data from the
15 Alzheimer's Disease Demonstration Grant Program grantees for a
period of twenty (20) months; and (2) disseminate program data
information at a national professional conference. The University of
Kansas is believed to be the only responsible source that has the
combination of experience and the capability to build upon earlier
work, in which they developed and performed all of the evaluation and
analyses of the Alzheimer's Disease Demonstration Grants to States
Program. The University of Kansas' critical involvement in data
development, data collection and data management since the inception of
the Alzheimer's Disease Demonstration Grants to State program further
qualifies them as the only responsible source. The purpose of this
synopsis is to comply with the notice requirement of FAR 5.201. Any
organization that believes it is capable of performing similar services
should submit a statement of capabilities with documentation supporting
its ability to meet the Government's requirement as described above.
Capability statements should be submitted within 45 days of the
publication of this synopsis to the contact point and address given
