combined synopsis/solicitation. Request for Quotation #RFQ-618-143-99
entitled "Percutaneous Vascular Surgical Closure System" is hereby
issued for commercial items to be used in the VA Cardiology Department.
and is prepared in accordance with the format in FAR Subpart 12.6 as
supplemented by additional information included in this notice and
incorporates provisions and clauses in effect through FAC 97-11. The
SIC code for this is 3841 and the small business size standard is 500
employees or less. This CBD announcement constitutes the only
solicitation issued. A separate written solicitation document will not
be issued. Contractors' proposals are hereby being requested and
response to requirements listed below shall be submitted on
contractor's letterhead or bid form and must address all requirements
listed and must contain the name, title and signature of person
authorized to submit proposals/bids on behalf of the contractor.
Descriptive literature for each item offered must also be includedin
proposal/bid package. Representative samples or system demonstrations
may be requested by the Contracting Officer at her discretion and will
be due within 5 days of request. The VA Medical Center (VAMC)
Minneapolis, Minnesota, has a requirement for a percutaneous vascular
surgical closure device/system that will percutaneously, surgically
close the femoral artery site of femoral sheath insertion following
either an angiogram or a PTCA procedure. The items as listed are
manufactured by Perclose Inc. of Menlo Park, CA or EQUAL. VAMC yearly
requirements are estimated as follows: (1) #TXL-431-06 -- Techstar XL
6FR Percutaneous Vascular Surgical System includes 1 Techstar XL PVS
Device and 1 perclose knot pusher -- Estimated Yearly usage is 75
Each/year. (2) #PXL-431-08 -- Prostar XL 8FR Percutaneous Vascular
Surgical System includes 1 Prostar XL PVS Device and 1 perclose knot
pusher, Estimated yearly usage is 145 Each/Year. (3) #PXL-431-10 --
Prostar Plus 10FR Percutaneous Vascular Surgical Systemincludes 1
Prostar Plus 10 PVS Device and l perclose knot pusher -- Estimated
Yearly usage is 50 Each/year. (4) #PAT-301-03 Arterial Tamper, an
accessory for the perclose PVS system -- Estimated Yearly usage is 20
Each/Year. (5) #PSH-331-01 Perclose Knot Pusher, an accessory for the
perclose PVS System -- Estimated Yearly usage is 20 Each/Year. The VAMC
is looking for a system that will provide the following benefits to our
cardiology patients and personnel: securely close the femoral artery;
shorten total patient bed rest time to as little as 3 hours; greatly
improve patient comfort; and lessen the need for pain medication;
impact and greatly reduce the use/need for the femostop and arterial
monitoring systems; lessen the need for nursing overtime; and shorten
length of stay for patients in the medical facility after angiogram or
PTCA procedures. It is anticipated that the VA Medical Center
Minneapolis will enter into a one year contract agreement with four
12-month options to renew for a total contract period not to exceed
five years. Delivery terms shall be FOB DESTINATION (unless vendor
states otherwise) to the VA Medical Center, Warehouse Bldg. 70 (90D),
One Veterans Drive, Minneapolis, MN, 55417. This will be a firm fixed
price contract. Prices bid will be in effect for one year at a time.
Contractor may offer differing prices for each of the option years.
Option Year pricing may be bid at separate prices (either higher, lower
or the same) from previous years. The following incorporated FAR
clauses and provisions are those in effect through FAC 97-11 and are
applicable to this solicitation: 52.212-1 Instruction to Offerors --
Commercial Items; 52.212-2 Evaluation -- Commercial Items; 52.212-3
Offeror Representations and Certifications-Commercial Items (NOTE:
Offerors must include a completed copy of "Offeror Representations and
Certifications" along with his offer); 52.212-4 Contract Terms and
Conditions-Commercial Items; 52.212-5-Contract Terms and Conditions
Required to Implement Statutes on Executive Orders-Commercial Items and
Y2K Warranty Clause. All responsible sources who can meet the VAMC's
requirements and provide items as listed above may respond to this
solicitation by submitting cost and technical proposals on their
company letterhead or bid form. Technical capabilities of the
contractor will be considered along with price proposal to determine
"best value" to the Government. Technical proposal must furnish
detailed data and information concerning technical capability of the
items offered to meet the governments requirements, past performance
history of the contractor (including case reports and 3 references with
names and phone numbers) Descriptive literature for all items offered
must accompany all proposals submitted. Evaluations may be conducted to
determine that products offered will meet Government's requirements.
Evaluation and award of a contract may be made with or without
discussions with the offeror(s). Submit all price and technical
