synopsis/solicitation for commercial services prepared in accordance
with the format in Subpart 12.6, as supplemented with additional
information included in this notice. This announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. This is Request for Proposal
N66604-99-R-2807. This solicitation and incorporated provisions and
clauses are those in effect through Federal Acquisition Circular 97-11.
This acquisition is a 100% set-aside for small business. The Standard
Industrial Code (SIC) is 8322; the size standard is $5M. The Naval
Undersea Warfare Center, Division Newport, Rhode Island (NUWCDIVNPT)
intends to purchase the on-site, full-time services of a Licensed
Clinical Social Worker for Naval Air Station, Family Service Center,
Brunswick, Maine. Direct clinical services are required until 30
September 1999. The Navy Family Service Center (FSC) exists to provide
service members and their families with services and skills for
self-sufficiency, professional and personal success, to facilitate
personal and family readiness, and to assist with adaptation to Navy
life. These skills and services translate directly to operational
readiness for commands by promoting good professionalism, healthy
lifestyles, financial management skills, strong family units, self
confidence and sound mental health. The contractor shall provide
Licensed Clinical Social Workers that possess a current, valid Maine
clinical license permitting him/her/them to function as an independent
health practitioner in the State of Maine. The contractor shall
provide direct clinical services and maintain a caseload of 30 to 35
active cases. The contractor shall provide individual, marriage and
family, and crisis counseling in the brief therapy modality. The
contractor shall provide counseling to children only when a parent is
present, unless the contractor has documentation of current criminal
history in its background check. The contractor shall adhere to social
work ethics, standards of professional conduct, those standards
prescribed by the licensing board, and shall comply with Federal
statutes. When and if standards conflict, adherence to Federal statutes
and regulations shall supercede other requirements. These actions
include but are not limited to: the release of information; client
confidentiality; operational safety and mission security; and the duty
to warn potential victims of threats of death or serious bodily harm
made by clients under their care. Issues that could have an impact on
personal safety, operational safety or mission security shall be
reported to the Counseling Supervisor and the Family Service Center
Director within one working day. Throughout the duration of the
contract, the contractor shall not see FSC-eligible clients in private
practice. The contractor shall not see his/her own FSC clients
throughout the contract or for six months after termination of the
contract. Contractor personnel shall meet the following standards of
behavior: (1) present a neat, clean and professional appearance and
wear professional attire; (professional attire is defined to exclude
sneakers, athletic shoes, rubber boots, sweatshirts, sweat pants and
jeans); (2) be fluent in both written and spoken English; and (3) be
willing to learn about the Navy, and the Navy mission and lifestyle as
it impacts his/her clients. The contractor shall: adhere to the U.S.
Navy's no-smoking policy at the FSC facility; shall not deny clients
services based upon religious doctrine, personal preference, ethnic
background or appearance; and the values and beliefs of the contractor
shall not influence the alternatives offered to clients. The
contractor shall not make anti-military comments and/or present such
attitudes as witnessed by an overt demonstration of contempt or
presented as advice rendered to individual clients. Counseling may be
provided at the Family Service Center at Naval Air Station, Brunswick
between the hours of 8:00 am and 8:00 pm. Standard hours are from 7:30
AM to 4:00 PM but evening appointments are permissible. The Government
will supply all equipment and supplies. The Government will provide
administrative support. The contractor shall be responsible clinically
to the FSC Clinical Supervisor. The contractor shall meet for one hour
each week with the Clinical Supervisor in the FSC, at a time determined
by the Clinical Supervisor, for review and consultation regarding the
contractor's cases. The improper disclosure of private or sensitive
information by the contractor to any person or entity not entitled to
receive such information, or the failure to safeguard any privileged
information may subject the contractor to criminal liability. The
contractor shall not disclose or cause information to be disseminated
concerning the operation of the FSC which could result in, or increase
the likelihood of, a breach of either FSC or U.S. Navy security. The
contractor shall be governed by the requirements of the Federal Privacy
Act. The contractor shall not respond to any mediainquiries. The
contractor shall refer all media inquiries to the FSC Director within
one workday or as soon as possible, but not more than one day after the
inquiry. The contractor shall develop a client treatment plan
reflecting a presenting problem, objective data, clinical assessment of
the information provided, and a plan for treatment that reflects
military and civilian regulations and instructions, available
resources, and sound clinical practices. When the treatment plan
involves FSC counseling, the contractor shall advise the client on the
expected number of sessions to be conducted and on the policy
regarding maximum number of allowable sessions. The contractor shall
close a counseling case and complete a closing summary for the case
record when: (1) the treatment plan has been met; (2) the client is
referred to other agencies to assist them with problem resolution; (3)
the client terminates the counseling; (4) the counselor terminates the
counseling; and (5) the command terminates the counseling (in the case
of command referrals). Closures shall occur within 3 weeks of the last
contact. The contractor shall, when an "at risk" situation such as
potential threat to self or others (suicide, homicide, child abuse,
domestic violence, inability to care for self, sexual assault, etc.)
has been identified, provide an immediate and appropriate response in
100% of the cases. The FSC Director shall be notified immediately of
all "at risk" or crisis situations. The contractor shall, when risk to
the client or others is suspected, conduct a risk assessment in
accordance with guidance provided by Counseling Supervisor. The
contractor shall report all suspected child abuse and spouse abuse to
the Family Advocacy Representative (FAR) at the FSC. Non-emergency
verbal referrals to the FAR shall be provided by close of business the
day of client-contact with written follow-up referrals made within 2
working days of client-contact. All case files, statistics and
documentation shall be maintained in accordance with FSC Counseling
Division policy and procedures. The contractor must work within the
Navy system and have the ability to work in a clinical environment
where the safety and security needs of the Navy sometimes supercede the
clients' interests. The provision at 52.212-1, Instructions to Offerors
-- Commercial, applies to this acquisition with applicable addenda. The
solicitation will close on 10 June 1999 at 2:00 p.m. local time.

FAR Provision 52.212-2, Evaluation -- Commercial Items. Award will be
made to the offer determined to be the best value to the Government
based on the following factors: (1) Technical Capability -- resume. (2)
Past Performance -- references from up to 12 agencies or companies; may
go back 3 years but shall be the most recent contracts. The Government
will contact these references. The Government may consider references
not listed by the offeror. (3) Price. Quotation Content -- please
provide the following: Propose the hours that you intend to work during
this period of performance. Submit resume(s) for the full-time,
Licensed Clinical Social Worker (or LCPC, although LCSW is preferred).
Provide a list of references from up to 12 agencies or companies; may
go back 3 years but shall be the most recent contracts. References
listed shall include those entered into with Federal, state or local
governments as well as commercial customers. Offerors that are newly
formed entities without prior contracts should list contracts and
subcontracts as required above for all key personnel. Include the
following information for each: name of contracting activity or
commercial firm; contract number; brief narrative (less than 10 lines)
describing your involvement in the effort, complexity, objectives
achieved, and types of personnel who performed under the contract. List
point of contact and telephone number. Describe any quality awards or
certifications received. If the award or certification is over three
years old, present evidence that the qualifications still apply.
Propose your unit price and total amount to be billed. For instance, if
you bill bi-weekly, propose the bi-weekly price X 8 periods from 14
June to 30 September. Please do not propose to bill any sooner than
bi-weekly. Please advise whether or not your company will accept a
Government Master Card as payment. The Offeror shall include a
completed copy of the provision at 52.212-3, Offeror Representations
and Certifications -- Commercial Items, with its offer. The clause at
52.212-4, Contract Terms and Conditions -- Commercial Items, applies
with applicable addenda. Clause 252.212-7001 of the Defense FAR
Supplement (DFARS), Contract Terms and Conditions Required to Implement
Statutes or Executive Orders Applicable to Defense Acquisitions of
Commercial items (JAN 1997, applies with applicable addenda. The
